摘　　要：目的:探讨支撑钢板和超关节单臂外固定器联合应用治疗复杂胫骨平台骨折的疗效.方法:1997-2003年手术治疗复杂胫骨平台骨折37例,按Schatzker分型,Ⅳ型13例,Ⅴ型17例,Ⅵ型7例,采用支撑钢板和单臂外固定器联合应用治疗,术后35例获随访,术后1年行膝关节HSS评分.结果:35例经12～28个月的随访,骨折全部愈合,膝关节功能伸膝0°～5°,屈曲均超过100°,无膝关节不稳.1例发生切口感染,3例关节面出现Ⅱ期塌陷大于2 mm,膝关节HSS功能评分平均为85.3分,优25例,良8例,中2例,差0例,优良率94.3%.结论:支撑钢板和单臂外固定器联合应用治疗复杂胫骨平台骨折是一种可靠的方法,疗效佳.Objective： To study therapeutic effects of buttress plate combined with over-articular and single-armed external fixation for the treatment of complex tibial plateau fractures. Methods：From 1997 to 2003, 37 patients（male 24, female 13;with an average age of 38.2 years）with complex tibial plateau fractures were treated with buttress plate combined with over-articular and single-armed external fixation. According to Scbatzker classification, 13 patients were type Ⅳ, 17 type Ⅴ and 7 type Ⅵ. Thirty-five patients were followed up after operation and HSS score of knee joint were calculated 1 year after operation. Results：After followd-up from 12 to 28 months, all the fractures were healed after the treatment mentioned above. The extension function of knee joints were 0° to 5° and flexion exceeding 100°. There was no occurrence of unstable knee joint. Infection of incision occurred in 1 patient and collapse more than 2 mm on the articular surface at H stage of the treatment in 3 patients. The average HSS score of knee joint was 85.3. In this group,25 patients achieved excellent results, 8 good,2 poor and no bad. The excellent-and-good rate was 94.3%. Conclusion： Buttress plate combined with over-articular and single-armed external fixation is a reliable method for the treatment of complex tibial plateau fractures and can obtain good therapeutic effects.
