Twin-block矫治器治疗早期骨性II类错牙合的X线头影测量分析

摘　　要：目的:分析Twin-block矫治器治疗早期骨性II类错牙合的牙颌形态变化,并探讨其适应证。方法:21例早期骨性II类错牙合患者(5°<ANB<10°,以下颌后缩为主),男12例,女9例,平均年龄11.5岁(9～13岁),均戴用Twin-block功能矫治器进行一期治疗(平均8.6个月)后(以后牙建立咬合为准)行直丝弓矫治器二期治疗。一期治疗前后分别拍摄头颅定位侧位片,选择有代表性的测量项目进行测量,并对治疗前后的测量结果进行配对t检验和逐步回归分析。结果:治疗后SNB角、下切牙-下颌平面角(L1-MP)显著增大(P<0.05);ANB角、前牙覆盖(OJ)、上切牙-前颅底平面角(U1-SN)、上、下唇突点到E线距离(UL-E线、LL-E线)均显著减小(P<0.05)。前牙覆盖(OJ)减少量与治疗前ANB、(U1-SN)角呈正相关,r2=0.62;上唇突度(UL-E线)减小量与治疗前SNA呈正相关、与SNB呈负相关,r2=0.53;下唇突度(LL-E线)减小量与治疗前(U1-SN)角呈正相关、与(L1-MP)角呈负相关,r2=0.58。结论:Twin-block矫治器对尚处于生长发育期的早期骨性II类错牙合患者疗效显著,表现为骨改建与牙代偿的混合机制。对于下颌后缩显著、上切牙较为唇倾、下切牙较为直立的病例侧貌改善效果更佳。PURPOSE： To investigate the dentofacial morphological changes and the indication of Twin-block appliance for treatment of skeletal Class Ⅱ malocclusion. METHODS： Twenty-one patients （5°〈ANB〈10°, 12 males, 9 females） were recruited with an average age of 11.5 years.Lateral headfilm were performed for each patient before and after treatment with Twin-block appliance, Fifteen cephalometrics parameters were selected and measurement was performed. Paired t test and multiple regression analysis were used for statistical analysis. RESULTS： After treatment SNB（°）, （L1- MP） （°）, （Co-Go） （mm）, （Go-Gn） （mm）, （Co-Gn） （mm）, （Sn-A＇-UL） （°）, （LL-B＇-Pos） （°） increased significantly（P〈0.05） ; ANB（°）, WITS, OJ（mm）, （U1-SN）（°）, （UL-E line）（mm）, （LL-E line）（mm） decreased significantly（P〈0.05）. ANB（°） and （U1- SN）（°） before treatment were significantly related to reduction in overjet （y=-37.14＋0.43 ANB ＋0.36 （U1-SN）, r^2=0.62）; SNA（°） and SNB（°） before treatment were significantly related to reduction in （UL-E line） （mm） （y=-5.37＋0.44SNA- 0.37SNB, r^2=0.53）; （U1-SN） （°） and （L1-MP）（°） before treatment were significantly related to reduction in（ LL-E line） （mm） （y=-7.65＋0.18 （U1-SN）-0.09 （L1-MP）, r^2=0.58）. CONCLUSION： Twin-block appliance can be used successfully for early treatment of skeletal Class Ⅱ malocclusion. It is more indicated for patients with retruded mandible , more labially inclined upper incisors and more upright lower incisors.
