治疗儿童慢性鼻窦炎疗效及临床相关因素的探讨

摘　　要：目的探讨儿童慢性鼻窦炎治疗疗效及临床相关因素.方法 121例接受治疗的慢性鼻窦炎儿童,给予三阶段治疗:Ⅰ、系统、规范保守治疗1～2月;Ⅱ、腺样体肥大患儿予以刮除同时完成保守疗法;Ⅲ、上述方法治疗无效时,选择鼻内窥手术(Fess术),术后随访1年.结果在121例病人中(226侧),经保守治疗后治愈79例(65.3%),好转31例(25.6%),无效11例(9.1%);腺样体刮除+保守治疗31例,好转28例(90.4%),无效3例(9.6%);Fess术治疗14例,治愈8例(57.1%),好转2例(22.2%),无效1例(9.1%),失随访3例.结论儿童慢性鼻窦炎以规范、系统的保守治疗为首选,腺样体刮除是有效的辅助措施,Fess手术适应症应严格掌握.Objective To evaluate the results of therapy on pediatric chronic sinusitis and to analyze the relative factors restrospectively. Methods A total 12 children （226 sides） were treated through three periods： The disciplinary medical therapy was conducted about one to two months, including antibiotics, steroid, nasal, irrigation; The children with adenoid regetation needed to be carried out early;Once the above management failed, endoscopic sinus surgery （Fess） was necessary. Resuits Overall cases included 121 cases of chronic sinusitis, 77 males and 44 females, aged 5 to 17 years （average 14.8）, they underwent disciplinary management, the cure case were 79 （65.3 % ）, improved cases were 31, while in the other 11 cases （9.1% ） there was no effect. The total effective cases were 31 and patients with adenoidectomy were 28 cases （90.4%）, there was no effect in 3 cases. 14 patients were chosen to be treated Fess. Postoperative follow-up was over one year. Eight children were recovered （57.1% ）, improved cases were 2 （22.2 % ）, there was no change in 1 case （9.1% ）, 3 ：ases were not able to be contacted in follow-up. Conclusion Among the treatment for children with ：hronic sinusitis, the systemic, medical disciplinary management is considered first. Adenoidectomy is helpful effectively. Fess should be chosen strictly.
