摘  要:目的报道多层螺旋CT三维重建在髋臼骨折Letournel-Judet分型诊断及手术入路选择中的应用方法。方法髋臼骨折患者29例,均行骨盆前后位X线摄片,同时行骨盆髋臼多层螺旋CT扫描和三维重建,确定髋臼骨折Letournel-Judet分型。结果简单骨折18例,其中后壁骨折8例,后柱骨折3例,前壁前柱4例,横行骨折3例;复杂骨折11例,其中后壁加后柱4例,前壁加前柱2例,T型骨折3例,双柱骨折2例。根据三维重建结果,保守治疗6例,行手术治疗23例,均得到解剖或近似解剖复位;经随访髋关节功能优8例,良15例,优良率79.3%。结论多层螺旋CT三维重建在髋臼骨折Letournel-Judet分型治疗及手术入路选择中有重要指导作用。Objective To discuss the method of CT three-dimensional reconstruction in acetabulurn fractures and the Letoumel-judet clarification system. Methods Twenty-nine patients with acetabulurn fractures were investigated by X-ray and CT three-dimensional reconstruction. According to Letournel-judet classification system of fractures position and direction, 18 cases of simple fractures, including of posterior parietal fractures in 8 cases, posterior column fractures in 3 cases, anterior parietal column fractures in 4 cases, transverse fractures in 3 cases was diagnosed. The diagnosis with complicated fractures was 11 patients, including of posterior parietal and column fractures in 4 cases, anterior parieter and column fractures in 2 cases, T fractures in 3 cases, double column fractures in 2 cases. Results According to CT three-dimensional reconstruction, 16 cases underwent expectant treatment, 23 cases underwent operative treatment, with a results of anatomical or proximate to anatomical restitution in all of 23 patients. In the followed-up, there were 8 excellent , 15 good , the good-excellent rate was 79.3 %. Conclusion CT three-dimensional reconstruction have important effects for Letournel-judet classification treatment and operative approaches on acetabulum fracture.
