羟乙基淀粉与白蛋白对移植肾功能的影响

摘　　要：目的探讨肾移植术中6%的羟乙基淀粉(HES)与6%HES+20%白蛋白对移植肾早期功能影响。方法772例接受同种异体肾移植术患者,根据术中用HES和白蛋白治疗分为两组,A组(n=330):HES≥500 ml;B组(n=442):20%白蛋白≥100 ml+HES≥100 ml。以术后第1天血肌酐值/术前基础值≥75%为移植肾功能延迟恢复(DGF),比较DGF和急性肾小管坏死(ATN)发生率。结果DGF的发生率,A组31.2%,B组26.7%,P>0.05;ATN的发生率,A组7.6%,B组5.0%,P>0.05。结论肾移植术中用6%HES与6%HES+20%白蛋白对移植肾早期功能影响相当。Objective To observe the effect of 6 % hetastarch and 6 % hetastarch ＋ 20 % albutein on the early grafted kidney function in operation. Methods 772 cases were divided into two groups according to used HES and 20 % aibutein in opration：group A（n = 330） ： HES ≥500ml; group B （ n = 442 ） ： 20 % albutein ≥ 100ml ＋ HES 100ml, The ratio of creatine in the first day posttransplant and creatine before operation was ≥75 %, the graft kiney is named the delayed graft function（DGF）. The ratio of DGF and the ratio of ATN were observed. Results The ratio of DGF group A was 31.2 %, group B was 26.7 % ; The ratio of ATN group A was 7.6 %, group B was 5.0 % . The ratio of DGF and ANT was not significant in two groups（P 〉 0.05）. Conclusion The effect of 6 % hetastarch and 6 % hetastarch ＋ 20 % albutein on the early grafted kindey function in opration is similar.
