内镜鼻窦炎鼻息肉手术1167例临床分析

摘　　要：目的总结鼻内镜鼻窦炎鼻息肉手术的疗效以及相关影响因素。方法回顾性总结1167例慢性鼻窦炎鼻息肉患者,Ⅰ型1期81例,2期216例,3期67例,Ⅱ型1期54例,2期301例,3期205例,Ⅲ型243例。结果1167例内镜鼻窦手术中,术后并发症60例(5.10%),其中眶内血肿1例(0.09%),鼻腔粘连42例(3.60%),鼻中隔穿孔2例(0.17%),泪道损伤1例(0.09%),眶纸板损伤14例(1.20%)。术中出血量20￣300mL,平均45mL。总治愈率为84.60%,其中Ⅰ型治愈率为94.50%,Ⅱ型治愈率为84.30%,Ⅲ型治愈率为70.80%。经χ2检验,三型治愈率差异有显著性。结论正确的手术操作、术后规范化换药以及适当激素的使用是影响鼻内镜手术后疗效的主要因素。Summarize the clinic effects and the related factors of endoscopic sinus and polyp surgery. [Methods] Retrospective analysis of 1 167 patients of chronic sinusitis and nasal polyp. There were 364 patients of type Ⅰ , and 81,216, 67 cases respectively in level Ⅰ , Ⅱ, Ⅲ. 560 patients of type Ⅱ, and 54, 301,205 cases respectively in level Ⅰ , Ⅱ, Ⅲ and 243 patients of type Ⅲ. [Results] Complications appeared in 60 of 1 167 cases（5.1%） undergone the endoscopic sinus surgery. In which orbital hemtoma in lcase（0.09%）, adhesion of nasal cavity in 42（3.6%）, nasal septal perforation in 2 cases（0.17%）, dacryoduct damage in lcase（1.6%）, orbital paperacea damage in 14 cases（1.2%）. The blood loss in surgery was 20-300 mL, average 45 mL. The total cure rate of type I was 94.5%, type Ⅱ was 84.3%, type m was 70.8%. By X2 test, there was obvious difference in three types. [Conclusion] The main factors affecting the resuhs of endoscopic surgery, were follows： correct operation, regularly follow-up and suitable drugs.
