摘 要:目的了解浙江霍乱弧菌O139的耐药状况及毒力基因携带情况。方法采用WHO推荐的改良K-B法,进行了14种抗菌药物的耐药性测定;以多重PCR法对6种毒力相关基因(ctxA、rtxA、Ace、TcpA、Cri、Zot)进行检测分析。结果霍乱弧菌O139主要对SD、AMP、SM、COS、CAR、FURX耐药,耐药率均在76.7%以上;对FPA、CF、CIPX、AKN、较为敏感,敏感率均在94.5%以上;而且不同年份菌株的耐药情况有所不同。所有菌株均携带3种以上毒力基因,其中93.2%携带ctxA基因,86.3%携带全部6种毒力基因。结论霍乱的治疗和预防中应首选CF、AKN、FPA、CIPX等敏感抗菌药物;霍乱弧菌O139的毒力基因携带率高,致病性和流行性均强,所以今后的霍乱防制中应加强对霍乱弧菌O139监测与研究,及时发现和控制疫情蔓延。Objective To investigate drug resistance and situation of virulence genes of V. cholerae O139 in Zhejiang Province. Method The tolerance of V. chnlerae O139 to 14 kinds of antibacterial was determined by improved K-B method recommended by WHO, and 6 virulence genes (ctxA, txA, Ace, TcpA, Cri, Zot) were genotyped by multiplex PCR. Results V. chnlerae O139 mainly resisted SD, AMP, SM, COS, CAR, RURX, with all the resistant rates over 76.7%. But it was sensitive to FPA, CF, CIPX and AKN, with all sensitivity rates over 94.5%. However, strains of different years had different tolerance to drugs mentioned above. Genotype analysis showed that all the strains carried more than 3 virulence genes, among which, 93.2% carried ctxA; 86.3% carried all of 6 virulence genes. Conclusions The antibacterial such as CF, AKN, FPA and CIPX, might be the first choice in the treatment and prevention ngalnst cholera. Carrying the high virulence genes, V. cholerae O139 has strong pathogenicity and epidemiclty. So the monitory and investigation of V. chnlerac O139 should be emphasized in order to detect and control the epidemic in time.
