摘  要:目的:探讨盐酸青藤碱在治疗肾间质性纤维化中的作用及其发生机制,为临床如何阻止、防止肾小球和肾小管间质纤维硬化提供借鉴。方法:采用小鼠单侧输尿管结扎法诱导肾间质纤维化的动物模型,用盐酸青藤碱治疗,并设对照组、假手术组(小鼠每日按10m l/kg体重用生理盐水灌胃,直至治疗结束)、治疗组(选用正清风痛宁缓释片按每日30mg/kg体重给小鼠灌胃,连续3周,),检测血尿素氮(BUN)、血肌酐(SCr)、血清层粘连蛋白(LN)、纤维连接蛋白(FN)的含量,免疫组化法测肾脏组织转化因子β1(TNF-β1)的表达并进行肾脏病理学的观察。结果:血BUN、SCr进行3组比较,对照组BUN、SCr指标明显高于治疗组(P<0.01),略高于假手术组,但统计学比较差异无显著性意义(P>0.05);治疗组TNF-β1及FN指标均明显低于对照组和假手术组(P<0.01);LN亦较对照组和假手术组低(P<0.05)。对照组、假手术组肾小管结构消失,代之以弥漫性纤维组织增生及单核细胞浸润;治疗组虽肾小管萎缩、扩张,但病变较轻,绝大多数肾小管结构尚存,炎性细胞浸润也多呈局灶样分布。结论:盐酸青藤碱能降低BUN、SCr、FN、LN的含量以及TNF-β1表达,抑制间质纤维化,减轻肾损害,从而延缓肾脏疾病的发展。Objective : To explore the effect and happening mechanism of hdrechloric acid kukoline for the treatment of renal interstitial fibrosis, to supply some according for clinic how to resist glomeruli and nephric tubule interstitial fibrosis. Method: The mouse was carried out half nephric duct ligation to induce the renal interstitial fibrosis animal model, afterwards, to set up three groups, control group, sham operation group( intragastric administration with normal sodium according to 10ml/kg weight), treatment group( treatment with Zhengqingfengtongnin retard tablets three weeks), to examine the content of BUN, SCr, LN, FN, to determine TNF - β1 with immunity class method and observe the renal pathology. Result: BUN and SCr were contrasted among three groups, the control group was higher than treatment goup( P 〈 0.01 ), and also high a litter than sham operation group( P 〉 0.05 ) ; the TNF - β1 and FN in treatment group were lower than other two groups( P 〈 0.01 ), so did LN( P 〈 0.05 ), the nephric tubule construction was disappeared in the control and sham operation group instead diffuse fibroplasias and histoleucocyte soakage; the treatment group also existed the atrophic or distension of nephric tubule, but majority had total the construction of nephric tubule, only took on local inflammatory cel infiltrate. Conclusion : Hdrochloric acid kukoline can reduce the content of BUN, SCr, FN, LN and the express of TNF - β1, Resist interstitial fibrosis, lighten the renal damage, so that delay the development of renal diseases.
