摘  要:目的探讨低场MRI对早期股骨头缺血性坏死(ANFH)的诊断价值及其病理基础。方法对20例经病理或临床证实的ANFH患者的MRI征象进行分析。结果股骨头X线、CT无异常或CT轻微骨质异常改变,股骨头不变形、关节间隙正常者,MRI均为阳性,表现为线样或条带状T1WI低信号、T2WI高信号10个股骨头;呈片状、斑片状或不规则形T1WI低信号、T2WI高信号13个股骨头;呈小囊状TIWI低信号、T2WI高信号5个股骨头。压脂序列上病变相应区域均呈较高信号。结论低场MRI诊断ANFH亦具有高度敏感性,能准确反映坏死的部位及分期,可达到早期诊断的目的,为防止坏死股骨头塌陷提供治疗机会。Objective To assay the value of low field MRI in diagnosis of early avascular necrosis of femur head (ANFH) and its pathological condition was also analyzed. Methods The MR appearances were evaluated from 20 patients with pathological or clinical confirmed ANFH. Results In patients with normal hnctmgs on X- ray rams and normal presentations on CT scanning for femur head, or with mild bony abnormities on CT scanning, but with normal state of femur head and normal joint space on CT scanning, the MR appearances were found with ANFH. Ten of the femur heads were presented with linear or streak low intensity on T1WI, high intensity on T2WI; thirteen femur heads were found with patchy, dot or irregular low intensity on T1WI, high intensity on T2WI; five of the femur heads with small cystic low intensity on T1WI, high intensity on T2WI. High intensity of MRI were found in the corresponding disease locations. Conclusion Low field MRI has high sensitivity for early diagnosis of ANFH, and the accuracy of necrostic location and staging can help for early treatment of femur head collapse.
