替米沙坦对慢性心力衰竭患者血栓前状态的影响

摘　　要：目的观察替米沙坦对慢性充血性心力衰竭血栓前状态的影响。方法将45例慢性充血性心力衰竭患者随机分为替米沙坦组和对照组,替米沙坦组口服替米沙坦80mg,每天1次,共2周。对照组除不服用替米沙坦外,其他用药与替米沙坦相同。治疗前和治疗2周后测量2组血浆tPA、PAI活性,以及D-二聚体(D-D)和VWF水平。结果替米沙坦组和对照组在治疗前血浆tPA、PAI活性、D-二聚体(D-D)和VWF水平差异无显著性(P>0.05)。替米沙坦组治疗后血浆tPA活性显著性增高(P<0.05),血浆PAI活性、D-D和VWF水平均有显著性下降(P<0.05),而对照组却无显著性改变(P>0.05)。组间比较发现,替米沙坦组治疗后血浆PAI活性、D-D和VWF水平比对照组更低(P<0.05),而血浆tPA活性却无显著性增高(P>0.05)。结论短期口服替米沙坦可以明显改善CHF患者的血栓前状态,这可能与其降低CHF的栓塞危险和改善其预后有关。[Objective] To study the effect of Telmisartan on the prethmmbotic state in congestive heart failure （CHF）. [Methods] 45 patients in CHF were divided randomly into Telmisartan group（Micardis 80 mg qd×14 days ） and Control group, Control group did not administrate Telmisartan, the other drugs were same as Telmisartan group. Tissue plasminogen activator activity （tPA）, tissue plasminogen activator inhibitor-1 activity （PAI-1）, Plasma D- dimer （D-D） and von Willebrand factor （vWF） level were measured in 2 group CHF patients with heart function class Ⅱ to IV before and 2 weeks after administration. [Results] Plasma tPA, PAI-1, vWF and D-D level before drug administration were not significantly different between 2 groups （P 〉0.05）. Plasma tPA activity after drug administration in Telmisartan group was higher （P 〈0.05）, but Plasma PAI-1, vWF and D-D level were significantly lower （P 〈0.05）, and the level of above all indexes in control group were not significantly decreased after administration （P〉0.05）. Compared with control group, plasma PAI-1, vWF and D-D level after drug administration in telmisartan group were significantly lower （P 〈0.05）, but plasma tPA activity was not significantly increased （P 〉 0.05）. [Conclusion] Telmisartan can improve the prethrombotic state in congestive heart failure, which may be correlated with decreasement of thrombembolia and death risk in CHF patients.
