绝经前后妇女性激素六项与骨质疏松的关系

摘　　要：目的探讨绝经前后妇女性激素6项与骨质疏松的关系,为指导骨质疏松患者的性激素替代治疗提供科学的依据。方法88例妇女分成绝经前期(1组)、围绝经期(2组)和绝经后期(3组)三组,分别抽血测定性激素6项:雌二醇(E2)、催乳素(PRL)、促卵泡成熟素(FSH)、促黄体生成素(LH)、孕酮(P)、睾酮(T),并于当月测量右跟骨定量超声。结果1组E2:(98.32±11.64)Pg/ml、T:(0.39±0.03)ng/ml、BUA:(66.3±4.6)dB/MHz、SOS:(1 527±26)m/s、SI:(91±10)%。2组E2:(47.27±6.62)Pg/ml、T:(0.25±0.02)ng/ml、BUA:(64.7±3.8)dB/MHz、SOS:(1 499±28)m/s、SI:(82±13)%,与1组比较除BUA外差异均有显著性意义(P<0.01)。3组E2:(9.46±1.4)Pg/ml、T:(0.12±0.01)ng/ml、BUA:(54.2±5.1)dB/MHzS、OS:(1 470±47)m/s、SI:(60±12)%,与2组比较差异均有显著性意义(P<0.01)。结论妇女绝经后骨质疏松症的发生、发展与血性激素6项中E2及T的进行性下降有密切的关系,治疗上宜采用补充雌激素为主结合补充孕激素和雄激素的性激素替代疗法。Objective To investigate the relationship between osteoporosis and sex hormone of menopausal women. Methods 88 womens were divided into 3 groups, Group 1 consisted of premenopausal women, Group 2 consisted of peri - menopausal women and Group 3 consisted of postmenopansal women. The levels of Estradiol （E2）, Prolactin （PRL）, Prolan B （PB）, luteinizing hormone （LH）, Pro - gesterone （P）, and Testosterone（T） were tested. Broadband ultrasound attenuation（ dB/MHz）, speed of sound （m/s）, and stiffness index （ % ） were measured at the right calcanea of the women. Results The level of E2, T, BUA, SOS and SI were 98.32± 11.64, 0.39 ±0.03, 66.3± 4.6,1527± 26 and 91±10, showing significant difference as compared with that of Group 2 except the level of BUA （E2：47.27 ± 6.62,T：0.25± 0.02,BUA：64.7 ±3.8,SOS：1499 ± 28,SI：82 ± 13） （P 〈 0.01） ; The level of E2, T, BUA, SOS and SI were ：9.46±1.4, 0.12 ±0.01, ：54.2 ± 5.1, 1470 ± 47, 60 ±12, showing significant difference as compared with that of Group 2 （ P 〈 0.01 ）. Conclusion The occurrence of osteoporosis in menopausal women is associated with the progressive reduction of sex hormone E2 and T. Supplement of E2, combined with P and T, could be used for treatment of osteoporosis in menopausal women.
