非浓缩尿蛋白电泳联合尿微量蛋白及尿酶诊断高血压病早期肾损伤

摘　　要：目的:探讨非浓缩尿蛋白电泳联合尿微量蛋白及尿酶诊断高血压病早期肾损伤的价值。方法:采用法国Se-bia公司Hgdrasys全自动电泳仪进行非浓缩(SDS-AGE)尿蛋白电泳,采用Beckman-CX7全自动生化分析仪,以终点法检测尿微量白蛋白(mAlB)和N-乙酰-β-D氨基葡萄糖苷酶(NAG),速率法检测尿肌酐(Cr)。结果:高血压病肾损害早期尿蛋白电泳多出现肾小管性蛋白,正常对照组尿mAlB/Cr和NAG/Cr结果分别为(1.42±0.93)mg/(mmol.L)和(4.83±2.31)U/g,尿蛋白定性阴性组高血压病患者尿mAlB/Cr和NAG/Cr较正常对照组增高(P<0.01),尿蛋白定性阳性组高血压病患者尿mAlB/Cr和NAG/Cr较阴性组增高(P<0.01),单项或双项检测SDS-AGE尿蛋白电泳,mAlB或NAG这3项指标阳性率偏低,将这3项指标联合检测阳性率高达85.71%。结论:非浓缩尿蛋白电泳联合尿微量蛋白及尿酶检测是诊断高血压病早期肾损伤的敏感方法。Objective： To investigate the value of combination of non-concentrated proteinuria electrophoresis urinary microalbumin（mALB） and urinary N-acelyl-β-D-glucosaminidase （NAG） for diagnosis of early renal disease in hypertensive patients. Methods：One hundred and twenty four cases of hypertensive patients were randomly divided into negative and positive groups of urine protein according to urine protein test. Sodium dodcyl sulfate-agarose gel electrophoresis（SDS- AGE） was used to analyze the proteinuria, mALB and NAG were determined with endpoint colorimetric analysis. Urine creatinine with Jaffe＇s rat. Results： The tubular proteinuria was more found in early renal disease from hypertension by SDS-AGE than other types. The levels of mALB/Cr and NAG/Cr in the controls were 1.42±0.93 mg/（mmol · L） and 4.83±2.31U/g . The levels of mALB/Cr and NAG/Cr in negative groups of urine protein with hypertension were higher than those in the controls（P〈0.01）. The levels of mALB/Cr in positive groups of urine protein with hypertension were higher than those in the negative groups （P〈0.01）. The positive rates of determining one or two of the three items were lower than that of determining SDS-AGE, urine mALB and NAG together. Conclusion： The method is sensitive to determine SDSLAGE, urine mALB and NAG together for diagnosis of early renal disease in hypertensive patients.
