吻合器痔上粘膜环切术治疗重度痔病226例疗效分析

摘　　要：目的探讨重度痔病应用吻合器痔上粘膜环形切除的临床效果。方法对我科自2001年10月～2005年3月收治的226例重度痔病患者施行了PPH术的病例进行回顾性分析。作者下移了荷包缝合位置,改进了PPH手术技术。结果226例重度痔病患者行PPH术的平均手术时间为15min,平均住院时间为3.5d,经治疗后全部病例痔核回缩、消失满意率达95%,基本满意率为5%。术后患者均有不同程度的肛周疼痛,其中102例需用止痛药物止痛。本组患者无肛周感染、肛门狭窄、大便失禁和直肠阴道瘘的发生。2例出现术后出血需要再次手术8字缝合止血。术后随访6个月至3年,效果良好,仅有1例患者术后2年出现复发。结论吻合器痔上粘膜环形切除是治疗重度痔病的有效手段,具有手术时间短、恢复快、并发症少、复发率低等优点。severe hemorrhoids Objective To investigate the clinical effect of PPH method in the treatment of Methods From October 2001 to March 2005, 226 cases with severe hemorrhoids received PPH were analyzed retrospectively. Results The average operation and hospitalization time was 15 minutes and 3.5 days respectively. After treatment, all the hemorrhoids were retracted completely, and the disapeared satisfactory rate was up to 95%, the primary satisfactory rate was 5%. Postoperative pain of different degree was found in the all cases. However, the analgesic application was needed in 102 cases. No Perianal infection, anal stenosis, fecal incontinence and rectovaginal fistula were occurred after operation. Postoperative local bleeding was found in two cases. With follow-up from 6 to 36 months, the efficacy was excellent and good. Homorrhoidal recurrence was happened only in 1 case two years later. Conclusion PPH is an effective measure for the treatment of the severe hemorrhoids. It＇s advantages are shorter time of operation, quicker recovery, less complications and a lower recurrence rate.
