彩色多普勒超声心动图诊断胎儿先天性心脏病的临床价值

摘　　要：目的:探讨彩色多普勒超声技术在诊断胎儿心脏异常中的应用价值。方法:对临床高度疑有胎儿先天性心脏病(CHD)的孕妇2600例行胎儿超声心动图检查。结果:检出胎儿心脏异常20例,检出率0.77%。其中CHD17例(0.65%),心脏肿瘤1例(0.038%),三尖瓣中量反流1例(0.038%),心包少量积液1例(0.038%)。经引产尸体解剖证实11例,产后彩色多普勒超声检查证实7例,失访2例。结论:胎儿超声心动图检查对产前检出胎儿心脏病具有重要的临床价值;胎儿心胸或心腔比率异常及胎儿心脏四腔观异常是胎儿CHD的最主要声像图表现。Objective ：To assess the value of echocardiography in diagnosing abnormality of fetal heart,especially of fetal congenital heart disease （CHD）. Methods： The study included 2600 pregnant women who were at high risk of CHD. All fetuses were examined using color Doppler echocardiography. Results： Twenty cases of fetal cardiovascular abnormality were found, with the abnormality rate of 0. 77%. Seventeen cases were CHD,with the detection rate of 0. 64%. One was heart tumor, one moderate regurgitation of tricuspid valve and one mild pericardial effusion. Among these cases, 11 were confirmed by autopsy, 7 were compared with postnatal echocardiography, 2 were lost to follow up. Conlclusion ： Echocardiography is of great value for prenatal diagnosis of fetal CHD. The abnormalities of the eardiothoracic ratio, heart chamber ratio and four-chamber vision are most important images of fetal CHD.
