血小板源性CD40L、CD62P在哮喘患者外周血中的表达及其临床意义

摘　　要：目的探讨血小板源性CD40配体(CD40L)、血小板P选择素(CD62P)在哮喘患者外周血中的表达及其临床意义。方法体外分离哮喘患者的血小板,用流式细胞仪测定二磷酸腺苷(ADP)诱导前后血小板源性CD40L、CD62P的表达,并与健康人比较。同步测定所有受试者的肺功能,分析与CD40L、CD62P表达的关系。结果①哮喘急性发作组(n=37)、哮喘缓解组(n=22)、健康对照组(n=13)血小板源性CD40L的阳性表达率分别为:(20.1±7.3)%、(5.2±3.1)%、(4.1±2.2)%,CD62P分别为(46.0±11.1)%、(12.6±4.8)%(、10.0±2.7)%;ADP诱导20分钟后CD40L的阳性表达率分别为:(19.7±7.6)%(、20.1±5.6)%(、25.4±12.8)%,CD62P分别为(45.5±11.4)%、(45.5±10.5)%(、43.2±15.4)%。与健康对照组、哮喘缓解组相比,哮喘急性发作组CD40L、CD62P的表达水平显著升高(P<0.05),而ADP诱导后其差异无统计学意义(P>0.05)。②健康对照组血小板源性CD40L、CD62P的表达在ADP诱导20min时达到最高水平,且CD40L、CD62P表达水平呈正相关(P<0.01)。③哮喘急性发作组FVC占预计值百分比(FVC%Pred)、FEV1占预计值百分比(FEV1%Pred)、FEV1占FVC百分比(FEV1%)较对照组显著降低(P<0.05),哮喘患者血小板源性CD40L和CD62P的表达水平与其肺功能成负相关(P<0.05)。结论哮喘急性发作患者存在外周血血小板源性CD40L、CD62P的表达增高,表达水平与其肺功能成负相关,血小板源性CD40L在哮喘的发病中起着一定的作用。Objective To investigate the expression of platelet-derived CD40 ligand （CD40L）, Pselectin（CD62P） in peripheral blood and its clinical significance in asthma. Methods Platelets were isolated from patients with asthma in vitro,the expression of platelet-derived CD40L,CD62P were measured by flow cytometry before and after platelets were induced by adenosine diphosphate （ADP）. Meanwhile the pulmonary function of all participants were evaluated, the relationship between pulmonary function and CD40L,CD62P was analysed. Results ①The expression of CD40L in patients with acute asthma group（n=37）, stable Ratients group （n=22）, control group （n=13） were （20.1±7.3）%, （5.2±3.1）%,（4.1±2.2 ）% respectively,and （19.7＋7.6）%,（20.1＋5.6）%,（25.4±12.8）% after ADP induced for 20 min; CD62P was （46.0±11.1）%,（12.6±4.8）%,（10.0±2.7）% respectively,and （45.5±11.4）%, （45.5±10.5）% ,（43.2±15.4）% after induced with ADP for 20 min. The expression of CD40L and CD62P were significantly increased in patients with acute asthma as compared with control group and stable patients group（P 〈0.05） ,but no difference after induced with ADP （P〉0.05）.② The expression of platelet-derived CD40L and CD62P reached their maximum level after induced with ADP for 20mira Furthermore, the levels of CD40L, CD62P were significantly positively correlated with inducing time extending （P〈0.01）.③In comparison to control group, FVC% Pred, FEV1% Pred and FEV1% in patients with acute asthma were significantly declined（P〈0.05）. The expression of platelet-derived CD40L and CD62P in patients with acute asthma was negatively correlated with their pulmonary function. Conclusions These results suggested there was high level expression of platelet-derived CD40L and CD62P in patients with acute asthma, the ievel was negatively correlated with the pulmonary function, plateletderived CD40L had a potent effect in the pathogenesis of asthma.
