摘  要:目的观察高压氧(hyperbaric oxygen,HBO)治疗缺血性脑卒中对血清中细胞粘附分子的影响。方法采用完全随机分组,将脑卒中患者分为137例HBO治疗组、97例常规治疗组,并以40例健康人作为正常对照组,用美国R&D Sys-term公司EL ISA试剂盒检测血清可溶性细胞间黏附分子1(sICAM-1)和可溶性血管细胞黏附分子1(sVCAM-1)水平。结果常规治疗组sICAM-1和sVCAM-1水平开始时比正常对照组高,差异显著(P<0.01);第10 d仍高于对照组,差异显著(P<0.05);第20 d恢复正常,无显著性差异(P>0.05)。高压氧治疗组sICAM-1和sVCAM-1开始时比正常对照组也高(P<0.01);第10 d、第20 d与对照组相比无显著性差异(P>0.05)。结论HBO治疗能通过调节细胞黏附分子的表达水平,降低缺血灶炎症反应的程度,对减轻缺血性脑卒中脑组织的损伤,预防复发有显著效果。Objective To observe the effects of hyperbaric oxygen(HBO) on adhesive molecule levels in patients with cerebral ischemic stroke,investigate the mechanism of HBO in treating the stroke. Methods Used ELISA kit from USA to observe the levels of serum soluble intercellular adhesive molecules (sICAM-1) and soiuble vascular cell adhesion molecule (sVCAM-1) in these three groups:137 patients in HBO group, 97 patients in routine therapy group and 40 cases in normal control group in the same period. Results Compared with the normal control group,the levels of serum sVCAM-1 and sICAM-1 in routine therapy group were higer in the first day and the 10th day (P〈0. 01) ;there was no significant difference in the 20th day (P〉0. 05). While the levels of serum sVCAM-1 and sICAM-1 in HBO group were higher in the first day (P〈0.01),but there was no significant difference in the 10th day and the 20th day (P〉0. 05). Conclusion HBO treatment may alleviate the damage caused by restraining the expression of adhesion molecules. That may be one of the therapeutic mechanisms.
