婴幼儿烧伤合并轮状病毒肠炎12例的治疗

摘　　要：目的 探讨婴幼儿烧伤合并轮状病毒肠炎的临床特点及治疗方法。方法回顾分析近4年医院收治的12例烧伤合并轮状病毒肠炎患儿的病史、临床特征及诊治经过。结果12例均为秋冬季节发病，腹泻物为蛋花汤样或稀水样便，伴有不同程度的发热、呕吐、腹胀；其中6例（50％）有腹泻患儿接触史；易引起医院感染流行；腹泻发病时间：入院前3例，入院后1～5d1例，5～lOd2例，10～20d2例，≥20d4例，6例开始感染时仅表现为急性呼吸道感染症状，2～4d后出现腹泻；病程为3～lOd；烧伤创面经保痂、换药、抗感染及手术等综合治疗，创面愈合出院。结论秋冬季节烧伤的婴幼儿出现蛋花汤样或稀水样便，大便常规无红、自细胞要高度警惕轮状病毒肠炎的发生；隔离、加强无菌操作是防止感染扩散的有效措施；止泻、补液、防止创面感染是治疗的关键。OBJECTIVE To look for a better approach to burned infants complicated with rotavirus diarrhea. METHODS Twelve cases of burned infants with diarrhea happened within autumn or winter in the recent 4 years were analyzed. RESULTS All of them with watery diarrhea were also accompanied with fever, vomiting, nausea, and distention. Six of them （50%） had a contact history with infants with diarrhea. From 12 cases the diarrhea of 3 cases happened before admission , 1 case in 1-5 days after admission, 2 cases in 5-10 days after admission, 2 cases in 10-20 days after admission, and 4 cases happened more than 20 days after admission, six cases showed acute respiratory symptom at first, then after 2-4 days the diarrhea came, experienced a total 3-10 day duration. All of them were treated well with comprehensive approach including antibiotics, antidiarrheal, oral rehydration or intravenous fluids, protection of burned area from contamination and so on. CONCLUSIONS During autumn or winter, rotavirus enteritis be highly on alert, the burned infants are accompanied with a watery diarrhea and no red or white blood ceils being found in the stool analysis. Isolation of the patients, and sterilized operation are most important to prevent infection spreading. Antidiarrheal, supplement of the lost liquid, protection of the burned area from contamination are the key points to guarantee a successful treatment.
