摘  要:目的探讨大黄对创伤性颅脑损伤患者血清炎性细胞因子的早期影响及其临床意义。方法将42例闭合性颅脑损伤患者随机分为大黄治疗组(22例)和对照组(20例),采用双抗体夹心酶联免疫吸附法(ELISA),检测2组患者治疗前及治疗244、8h后血清肿瘤坏死因-子a(TNF-a)、白细胞介素(IL)-2I、L-6I、L-8水平,同时检测10名健康者作为正常组。结果2组脑创伤患者血清TNF-aI、L-2I、L-6I、L-8水平显著高于正常组(P<0.05);大黄治疗后,血清IL-2I、L-6I、L-8水平明显下降(P均<0.05);而TNF-a无显著变化。结论大黄能显著降低创伤性颅脑损伤患者早期血清IL-2I、L-6I、L-8水平。AIM To investigate the therapeutic effects of the Chinese medical herb, Rhubarb, on the serum levels of inflammatory cytokines in patients with traumatic brain injury (TBI) and study their clinical meaning. METHODS The 42 patients with TBI were randomly divided into 2 groups: the Rhubarb treatment group ( n = 22) which Rhubarb powder (12- 24 g·d^-1) was injected into every patient's stomach by tube besides routine treatment; and the control group ( n = 20) which the only routine treatment was applied to. By enzyme-linked immunoadsordent assay(ELISA), tumor necrosis factor-alpha(TNF-a), interleukin-2, interleukin-6 and interleukin-8 in serum were analyzed before the treatment and at 24 h, 48 h after the treatment. Ten healthy people were as normal control. RESULTS Compared with healthy people, TNF-a, interleukin-2, interleukin-6 and interleukin-8 in serum of all patients with TBI were significantly increased after trauma (all P 〈 0.05). Rhubarb could decrease the serum levels of IL-2, IL-6 and IL-8 significantly, in especial IL-6, compared with that before the treatment or of the control group ( all P 〈 0.05). But the level of TNF-α couldn't be reduced. CONCLUSION Inflammatory cytokines play an important role in secondary brain injury following TBI. Rhubarb can inhibit releasing of inflammatory cytokines and decrease the serum levels of cytokines.
