Ebselen及二乙基羟基纤维素在急性胰腺炎病人肠内营养中的应用

摘　　要：目的验证在肠内营养中加入Ebselen和二乙基羟基纤维素(ethylhydoxyehtyl cellulose,EHEC)是否可以减轻急性胰腺炎(AP)病人的急性期反应和多器官功能障碍的程度。方法对AP病人应用Ebselen和EHEC的混合物,并以健康志愿者做对照,观察AP病人的病情严重程度和血浆炎症介质的表达量。结果在AP病人的肠内营养中加入Ebselen和EHEC的混合物可以减少病人发热的发生,降低抗生素的应用时间、禁食时间、住院时间和病死率。结论肠内营养中联合应用Ebselen和EHEC是治疗急性重症胰腺炎的一种新型有效的方法。【Objective】 To investigate whether the supplement of ebselen and ethylhydroxyethyl cellulose （EHEC） to enteral nutrition can attenuate the acute phase responses and the severity of muhiple organ dysfunction in AP patients. 【Methods】 The compound of Ebselen and EHEC were supplied to the enteral nutrition with the AP patients, and the severity of the MODS and the plasma level of MIP-2, IL-6, MCP-1 and IL-1r were measured to evaluate the effect of the compound. 【Results】 Results from experimental stady show that the combination of ebselen and EHEC significantly reduced the occurrence of infection indicated by less frequency of fewer and antibiotic application, improved the gastrointestinal function by Short period of bowel opening, and altered the inflammatory responses by down regulation of circulating IL-6, MCP-1 and IL-1r in AP patients with single or multiple organ, and improved the outcome of AP patients by hospital stay and mortality in AP patients. 【Conclusion】 The combination of ebselen and EHEC may be a new potential and clinical benefit for treatment of acute severe panereatitis.
