六味地黄丸对激素和免疫抑制剂治疗系统性红斑狼疮干预作用的研究

摘　　要：目的:观察六味地黄丸对激素、免疫抑制剂治疗系统性红斑狼疮(SLE)的干预作用。方法:两组均给予泼尼松,首始剂量1mg/kg/日,早晨8点顿服,持续8周后,每2周减5mg,至30mg/日时,缓慢减量。有狼疮肾炎的患者,用环磷酰胺(CTX)0.8g静脉输注,每月1次。治疗组同时加服六味地黄丸8粒,每天3次。结果:治疗组总有效率96.67%,对照组有效率80.33%,治疗组疗效优于对照组(P<0.05);口腔溃疡、月经不调、肝功异常及补体C3、24小时蛋白尿等指标两组均改善显著(P<0.05,P<0.01),但以治疗组为优(P<0.05);其复发率以治疗组为低(P<0.05)。治疗组出现的阴虚火旺症候积分值及不良反应的发生率均较对照组为低(P<0.05,P<0.01)。结论:六味地黄丸能显著提高激素和免疫抑制剂对SLE的疗效、减少其复发,并能对抗激素的不良反应。Objective： To observe the intervention of Liuwei Dihuang pill （LDP） on therapeutic effectiveness and adverse reaction of hormonotherapy and immunosuppressive agent in treating systemic lupus erythematosus. Methods： Patients divided in two groups were medicated with initial dose of prednisone 1mg/kg once a day at 8 am in the morning. After 8 weeks, Medication the dose of prednisene was decreased by 5.0mg every 2 weeks till 30mg/kg per day. Then the prednisone was decreased gradually. The patient of lupus nephnitis were injected eyclophosphamide （CTX） 0.8g once a month. At the same time, the treated group was administrated LDP 8 capsules 3 times per day. Results： Therapeutic effect in the treated group was significant better than that in the control group 96.67%, 80.33% （P 〈 0.05 ） . Mouth ulcer, liver insufficiency, disorder of mensis and complement C3, 24-hour proteinura were improved （P 〈 0.05, P 〈 0.01 ） . However, as compared with the control group, the improvement was better, and the recurrent rate was lower （P 〈 0.05 ） in the treated group. Scores of Yin-deficiency caused excessive fire syndrome and incidence rate of adverse reaction in the treated group were lower than those in the control group （P 〈 0.05, P 〈 0.01 ） .Conclusions： LDP can markedly improve the therapentic effectiveness and counteract the adverse reaction of hormonotherapy and immunosuppressive agent in treating systemic lupus erythematosus, and reduce the recurrence of the disease.
