摘  要:目的探讨中性粒细胞趋化因子(CINC)在重症急性胰腺炎(SAP)发病机制中的作用。方法采用向胆胰管逆行注射5%牛磺胆酸钠建立大鼠SAP模型,72只雄性SD大鼠随机分为手术对照组(40只)和SAP组(32只),术后1、3、6、12h分别检测血清淀粉酶、胰腺组织病理学改变和肺干湿重比,免疫组织化学法检测胰腺和肺组织中CINC的表达。结果与手术对照组相比,SAP大鼠的血清淀粉酶明显增高,胰腺组织学明显改变。SAP组术后1h胰腺腺泡细胞即有CINC蛋白的表达,且与胰腺组织病理改变呈正相关。SAP组术后3h肺泡细胞有CINC蛋白的表达。结论CINC可能在SAP发病及其相关的急性肺损害(ALI)中起了重要的作用。Objective To investigate the role of cytokine-induced neutrophil chemoattractant (CINC) in the pathogenesis of severe acute pancreatitis (SAP). Methods The SAP model was induced by retrograde infusion of 5% sodium taurocholate into the bili-pancreatic duct in SD rats. Seventy-two SD rats were randomly divided into two groups: operational control group and SAP group. Serum amylase level, the pathological score of the pancreas and the dry/wet weight ratio of the lungs were measured at 1,3,6, or 12 h after operation. Expression of CINC was determined by immunohistochemistry in the pancreas and lung. Results Serum amylase and the histological score of SAP group were significantly higher than those of operational control group. CINC proteins in SAP group were detected in pancreatic acinar cells from 1 hour after operation, and positively correlated with pathological changes. CINC proteins in SAP group were detected in alveolar cells from 3 h after operation. Conclusion CINC may play an important role in the pathogenesis of SAP and acute lung injury (ALI).
