摘 要:目的观察吸入不同浓度的安氟醚对心肌酶及心肌肌钙蛋白的影响,探讨该药对心脏的影响。方法60例按美国麻醉医师学会分级为Ⅰ~Ⅱ级拟行气管插管全身麻醉手术患者随机分为Ⅰ、Ⅱ、Ⅲ、IV四组,分别吸入1%、2%、3%、4%的安氟醚。于吸人安氟醚前(Tn)、吸入安氟醚lh(T1)、2h(T2)、4h(T3)、吸入后24h(T4)取静脉血,测乳酸脱氢酶(LDH)、肌酸激酶(CK)、肌酸激酶同工酶(CK.MB)等心肌酶及心肌肌钙蛋白-ⅠI(cTn Ⅰ)等生化指标。结果四组吸入安氟醚后各时点上述各心肌酶及心肌肌钙蛋白与吸入安氟醚前比较无明显变化(P〉0.05),各时点组间比较无显著差别(P〉0.05)。结论临床常用浓度的安氟醚对心脏不会产生不利影响。Objective To evaluate the effects of enflurane on myocardial enzyme and cardiac troponin Ⅰ. Methods Thirty two ASA( American Society of Anesthesiologists, ASA) Ⅰ - Ⅱ patients undergoing elective surgery under general anesthesia with tracheal intubation were randomly divided into four groups of eight patients each: group Ⅰ , group Ⅱ, group Ⅲ and group Ⅳ. Enflurane with 1% ,2% ,3% and 4% concentration was inhated respectively.The vcin blood samples were taken before enflurane inhalation(T0), at the first hour( T1), the second hour( T2) and the fourth hour( T3) with enflurane inhalation and the twenty-four hour ( T4 ) after enflurane inhalation for determining the concenteation of blood plasma muocardial enzyme of lactic dehydrogenase (LDH), ereatine kinase( CK), CK isoenzyme( CK-MB) and cardiac troponin Ⅰ (cTnⅠ). Results In all the groups, the levels of LDH,CK,CK-MB and cTnI were not significant difference between before and after enflurane inhalation( P 〉 0.05). Meantime they were not significant difference between these groups in every time point. Conclusion The concentration of commonly applied enflurane inhalation does not result in disadvantageous heart effect in patients.
