摘  要:目的探讨双水平正压无创通气(BiPAP)对慢性阻塞性肺疾病(COPD)病人合并Ⅱ型呼吸衰竭的治疗价值。方法78例COPD病人合并Ⅱ型呼吸衰竭患者随机分成二组,治疗组38例和对照组40例,对照组给予常规抗感染、平喘、祛痰和低浓度氧疗,治疗组除了常规治疗外,加无创机械通气(BiPAP)治疗,比较两组患者治疗前后血气的变化,对结果进行统计学处理。结果治疗组治疗后动脉血气PaO2、SaO2逐渐上升,Pa-CO2值下降,与治疗前相比,有极显著性差异,P<0.01。对照组治疗后动脉血气PaO2、SaO2逐渐上升,PaCO2值下降,与治疗前相比,有显著性差异,P<0.05。比较两组治疗前后动脉血PaCO2下降幅度差,PaO2、SaO2上升幅度差,有显著性差异,P<0.05。结论本法治疗COPD病人合并Ⅱ型呼吸衰竭有肯定疗效。Objective To explore the treatment value of BiPAP in the COPD patients with type Ⅱ respiratory failure. Methods 78 COPD patients with type Ⅱ respiratory failure were randomly divided into two groups: study group (38 patients) and control group(40 patients). The patients in the control group are managed with the routine treatment,lncluding resisting infection, relieving dyspnea,eliminating phlegm and low-density oxygen therapy. The patients in the study group are managed with BiPAP in addition to the routine treatment. The arterial hlood gases were compared and analyzed statistically before and after treatment in the patients of the two groups. Results: The PaO2 and SaO2 in the study group of arterial blood gases goes up gradually while PaCO2 goes down after the treatment. Compared before treatment, we have an extremely contrasting difference, P〈0.01. PaO2 and SaO2 in the control group of arterial blood gases goes up gradually while PaCO2 goes down after the treatment. Compared before treatment, the difference is contrasting, P〈0.05 . Compared the decrease difference of PaCO2 and the increase difference of PaO2, SaO2 before and affter treatment in the patients of the two groups, P〈0.5. Conclusion: BIPAP was proved to be an effective therapy for the COPD patients with type Ⅱ respiratory failure.
