吻合器痔上粘膜环切钉合术的荷包定位与缝合技巧

摘　　要：[提要]采用美国强生公司痔吻合器手术治疗脱垂性痔172例,以痔核上缘作为荷包定位主要依据,原位粘膜下层双荷包缝合是本组手术操作要点。平均手术时间21min,所有脱出痔组织立即全部回缩,切除组织宽度平均3.6(2.1～4.6)cm,术后平均住院2.5d,术后非常满意者97例,满意者71例,基本满意者4例。随访1～42个月,无肛门失禁、脱出复发及吻合狭窄等。本方法作为PPH的荷包定位和缝合技术方法是有效的,值得临床推广。Objective 172 cases of prolapsed hemorrhoid underwent procedure for prolapse and hemorrhoids（PPH） with dedicated instrument （Johnson ＆ Johnson）. The purse string suture positioned right on the upper edge of hemorrhoid and double purse string stitch were employed in submucosal layer. Average operative duration was 21 minutes, average width of excised mucosal ring was 3.6 cm, and average hospitalization was 2.5 days. The prolapsed hemorrhoids went back right after surgery, and the outcome was categorized as ex- cellent in 97 cases, fairly in 71 cases and basic in 4 cases. No fecal incontinence, recurrence of prolapsed hemorrhoids and anastomotic stenosis occurred within 1 - 42 months follow-up. The positioning and suturing technique for purse string stitch is effective and recommendable.
