摘 要:目的了解吗啡依赖猴快速脱毒过程中血和脑脊液中β-内啡肽量的变化。方法吗啡依赖猴模型按剂量递增法(吗啡:0.6、1.2、1.8、2.4mg.kg-1.d-1)制备,除吗啡2.4mg.kg-1.d-1维持3周外,其余吗啡剂量均维持1周。吗啡依赖猴模型制成后,在全麻下分别用生理盐水作为对照组和用纳洛酮作为处理组进行脱毒,观察戒断症状并计算戒断症状分数。在脱毒的不同阶段取血和脑脊液,用放射免疫法定量检测β-内啡肽。结果吗啡依赖猴模型制备完成后,血和脑脊液中的β-内啡肽的水平与模型制备前相比显著降低(P<0.01);纳洛酮快速脱毒过程中,血和脑脊液中的β-内啡肽水平迅速升高(P>0.05),在给纳洛酮后2h达到峰值;对照组在观察的72h内出现戒断症状,处理组脱毒后连续观察3个月无戒断症状。结论增加体内β-内啡肽水平可能是全麻下纳洛酮对吗啡依赖猴快速脱毒的机制之一。Purpose To investigate the neuro-endocrine mechanism of detoxification by large dosage of naloxone under general anesthesia. Methods The model of morphine-dependent monkeys was developed by the method of increasing morphine dosage from 0.6 mg · kg^-1· d^-1 to 2.4 mg· kg^-1· d^-1 gradully. After the establishment of the model, detoxification on morphine-dependent monkeys with normal saline and naloxone under general anesthesia was separately performed and the efficacy was evaluated according to precipitated abstinence scores (PAS). Blood and cerebrospinal fluid was examined for the changes of β-endorphin throughout the whole experiment. Results Anesthesia itself has no effect of detoxificatioru Naloxone had a wonderful effect of detoxificatioru Monkey's β-endorphin was elevated swiftly after the administration of naloxone. Conclusions The possible mechanism of rapid opiate detoxification(ROD) on morphine-dependent monkeys by naloxone is the rapid increase of β-endorphin.
