血管紧张素转化酶抑制剂对化学性急性呼吸窘迫综合征预防作用的实验研究

摘　　要：目的研究早期应用血管紧张素转化酶抑制剂(ACEI)-卡托普利(CPT)对油酸致急性呼吸窘迫综合征(ARDS)的预防作用及其作用机制。方法36只大鼠随机分为CPT治疗组、ARDS组、假手术组。每组分别在静脉注药后3h、6h 2个时间点处死动物,左心室完全取血,测ACE、AⅡ、ET-1。测量左、右肺湿重,右肺干重,并观察左肺组织学改变。结果ARDS组肺水含量、血液中ACE、AⅡ、ET-1的水平较假手术组明显升高(P<0.05);CPT治疗组与ARDS组比较差异有显著性(P<0.05)。血液中ACE、AⅡ、ET-1的含量与相应的肺系数存在相关性(P<0.05)。结论在ARDS早期,血液中ACE、AⅡ、ET-1的水平与肺系数存在相关性,ACEI-CPT可抑制血液中ACE、AⅡ、ET-1水平的升高,对实验性大鼠急性呼吸窘迫综合征有一定的预防作用。Objective To study the preventive effects of ACEI-CPT on Wistar rats with ARDS induced by oleic acid, and to explor the possible mechanism. Methods 36 healthy Wistar rats were randomly divided into three groups：the CPT treatment group, the ARDS group, and the Sham operated group. The rats were killed 3h or 6h after operations respectively. The blood in the left ventricle was taken out completely to examine the contents of ACE,A Ⅱ and ET-1. The left and right lung tissues were weighed. The right part was desiccated and weighed again, the left part was prepared to be observed under microscope and electronic microscope. Results The water content in lung and the blood ACE ,A Ⅱ ,ET-1 contents in ARDS group were markedly increased when contrasted with the sham operation group （ P 〈 0.05）, and the difference between the CPT treatment group and the ARDS group was also quite significant（ P 〈0.05）. The correlation between levels of ACE,A Ⅱ , ET-1 and the corresponding lung coefficient was significant （ P 〈 0.05 ）. Conclusion In the early stage of ARDS, the blood ACE ,A Ⅱ ,ET-1 contents correlate with lung coefficients. The administration of ihbitior of angiotensin to convert enzyme captopril can inhibit the increase of their blood ACE,A Ⅱ, ET-1 levels. ACEI-CPT have certain good protective effects on rats with ARDS.
