摘  要:【目的】探讨近红外光谱技术对新生儿缺氧缺血性脑病脑氧合和组织灌注的评价,为临床评价脑损伤提供客观、量化依据。【方法】应用近红外光谱技术,测定脑损伤患儿的脑组织氧饱和度,并与对照组进行比较。对缺氧缺血性脑病的新生儿短暂吸低流量2 L/min的氧,并与对照组对照,观察吸氧后脑氧合和组织灌注的改变。【结果】缺氧缺血性脑病的新生儿脑组织氧饱和度为53%±3%,低于对照组的62%±4%,差异有显著性(P<0.05)。患儿吸氧后还原血红蛋白下降,氧合血红蛋白上升,脑组织氧饱和度上升,脑氧合及脑灌注的增加幅度小于对照组,两者之间差异有显著性(P<0.05)。【结论】在新生儿缺氧缺血性脑病时,脑组织氧饱和度能客观评价脑组织的氧合状态,缺氧缺血性脑病患儿存在脑氧合和脑灌注的异常。[Objective] To discuss the significance of the evaluation of brain oxygenation and perfusion of hypoxic ischemic encephalopathy(HIE) using near infrared spectroscopy (NIRS), and to provide a method to evaluate brain damage objectively and quantitatively. [Methods] Using NIRS, the brain regional oxygen saturation in HIE infants were measured, comparing with the control group. The newborn with HIE was administered an inhalation of low flux oxygen 2 L/ min, and compared with the control group, the evaluation of brain oxygenation and perfusion was discussed using NIRS. [Results] The brain regional oxygen saturation of HIE infants was 53 %± 3 %,while the control group was 62 %±4%, there were significant differences between them. The hrain oxygen saturation could be used to evaluate the brain oxygena tion objectively, the infant with HIE had abnormality in brain oxygenation and perfusion. After the inhalation of oxygen, Hb descended, HbO2 and rSO2 ascended,the change of brain oxygenation and perfusion was lower than that of the control group, there were significant differences between them. [Conclusion] In HIE infants, brain oxygen saturation can evaluate brain injury objectively, the infant with HIE had abnormality in brain cerebral oxygenation and perfusion.
