摘 要:目的探讨复方棒冰综合防治鼻咽癌患者放疗所致口腔黏膜反应的效果。方法将80例采用直线加速器放射治疗的鼻咽癌患者随机分为实验组及对照组各40例。对照组在整个放疗期间进行常规口腔护理:保持口腔卫生,每日于放疗前后及进食后漱口,必要时行雾化吸八;实验组除常规口腔护理外,从放疗开始每天于放疗前和饭前预防性地吃复方棒冰直至放疗结束后10d。监测两组放疗期间口腔酸碱度和唾液分泌量变化;记录患者进食情况,口腔黏膜变化,吞咽疼痛与放疗剂量的关系等。最后比较两组发生口腔黏膜反应的程度。结果放疗至30Gy时,两组疗效对比有统计学意义(P〈0.05),放疗至55~60Gy及放疗结束后lOd,实验组3~4级黏膜反应明显少于对照组(P〈0.01);实验组唾液分泌量及唾液pH值大于对照组,未发生不良反应。结论复方棒冰可用于综舍防治鼻咽癌患者放射性口腔黏膜反应。Objective To study the effect of compound popsicles against acute oropharyngeal mueositis caused by radiotherapy in patients with nasopharyngeal carcinoma (NPC). Methods Eighty patients with locoregionally advanced NPC were enrolled into this prospective randomized clinical trial. The cases were divided into treatment group (40 cases) and control group (40 cases). Radiation therapy was administered conventionally over 7 weeks in 2.0Gy daily fractions for 5 days each week to total doses of 70 Gy. All of the patients accepted oral care and gargled before dining and rediotherapy daily. Patients in control group accepted oral care conventionally. Patients in treatment group took the compound popsicles before dining and rediotherapy daily up to 10 days after finishing radiotherapy additionally. The oral pH and the saliva volume were monitored by a nurse twice every week. An objective mueositis assessment tool was used everyday to document the change of oropharyngeal mucus, food-intake, the degree of odynophagia of the patients. Mucositis were diagnosed and evaluated according to the WHO Toxicity Criteria. Results There was significant difference between two groups when total doses is 30 Gy (P〈0.05). The patients with mucositis (grade 3-4 adverse) in the treatment group was less than that in the control group when total doses were up to 55-60Gy and 10 days after radiotherapy. The difference between two groups is ,significant (P〈0.01). The oral pH and saliva vol- ume of patients in the treatment group were more than that of control group. No adverse effect of intervention was found in the two groups. Conclusion The compound popsicles were effective to reduce acute oropharyngeal toxicity resulted from radiothe〉 apy in patients with NPC.
