阿霉素心脏毒性160例心电图分析

摘　　要：本文报道160例在我科住院经病理确诊的癌症患者。治疗中采用以ADR为主的联合化疗,ADR 40mg/m^2快速静脉输注,每三周重复。用ADR前除9例原有心电图异常外,其余151例心电图均正常。用ADR达一定累积量时,原9例异常者变化不大,37例(37/151)出现了各种不同的心电图异常。表现心律失常11例,低电压14例;ST-T改变12例,其中4例发生了充血性心力衰竭。20例并用了环磷酬胺(CTX),14例有纵隔放疗史,与文献报道的并用CTX或/和胸部放疗是ADR所致心脏毒性的两个危险因素是一致的。In our department, 160 patients with cancer, who were pathologically provened, were treated with Adriamycin (ADR) based combination chemotherapy. ADR was given at the dose of 40 mg/m2, by rapid i.v. drip repeated every 3 weeks. 151 eases were normal in ECG with 9 cases abnormal, before ADR administration. When a certain accumulated dose of ADR was reached, 9 abnormal ECG were found, but the changes were not very great, but 37 cases (24.5%) showed various abnormal ECG, manifesting in arrhythmia (11), nonspecial ST--T changes (12), bw voltage (14), and congestive heart failure in 4 cases. Of which 20 cases administed with CTX and 14 cases combined with mediastinal irradiation this is in concordance with literatures that adminitration with CTX or/and combined with mediastinal arradiation is risk factors for cardiotoxicity.
