闭环刺激双腔频率适应性起搏器的临床应用及精神负荷测试观察

摘　　要：目的 观察闭环刺激双腔频率适应性起搏器对体力活动及精神负荷的反应。方法 对窦房结变时性功能障碍的16例患者，植入闭环刺激双腔频率适应性起搏器。植入后第2天开启闭环刺激频率适应性功能。有15例患者在传感器自动初始化后第6天进行算数法，其中3例于术后第9天进行倾斜试验法，以测试精神负荷下的闭环刺激频率适应性功能。术后1个月及半年，患者随访进行日常体力活动观察。结果15例患者在体力和脑力活动时，起搏频率均能按需增加。3例以算数法与倾斜试验法进行精神负荷测试对比者，2种方法起搏频率均可明显加快，倒计数法测试前平均起搏心率64次／min，测试中最高起搏频率平均87．7次／min（82～94次／min），倾斜试验测试前平均起搏心率65次／min，测试中最高起搏频率平均93．7次／min（86～105次／min）。结论闭环刺激双腔频率 适应性起搏器在日常体力活动和精神负荷时，可使起搏频率明显加快，满足生理需要。Objective To observe the responses of dual chamber rate adaptive pacemakers with closed loop stimulation （CLS） to physical activities or mental stress. Methods Dual chamber rate adaptive pacemaker with closed loop stimulation were implanted into 16 patients with sinus chronotropic disability. Rate adaptive function with closed loop stimulation were opened in the next day, Among them, 15 were dealt with the counting test ：n day 6 after automatic initializeation, 3 of them were dealt with tilt table test （TTT） to detect this function in day 9. Rate adaptive functions with closed loop stimulation were test under mental stress. Daily physical activities were test as patient follow-up one month and half a year later. Results Pacing rate could increase meeting the need when 15 patients were in physical activity or mental stress. It could also increase in 3 patients with counting test and ＇FIT for mental strcss assessment, and the aver age pacing rate touched 64 次/min while the top was 87.7 次/min （82 -94次/min）, and it was 65 次/min before TTT while the top was 93.7 次/min （86-105 次/min）, Conclusion The dual chamber rate adaptive pacemakers with closed loop stimulation can markedly increase, meeting the need for daily physical activities and mental stress.
