摘 要:目的检测血清铁(Serum Iron)、铁蛋白(Ferritin)和可溶性转铁蛋白受体(Soluble transferrin receptor,sTfe)等指标,并分析这些指标在诊断妊娠妇女缺铁性贫血中的临床意义。方法采用免疫分析法和比色法测定43例妊娠24~28周妇女和同期收集的30例健康妇女的血清铁、铁蛋白和可溶性转铁蛋白受体的水平,利用SPSS10.0软件对实验数据进行统计学分析。结果妊娠组血清铁、可溶性转铁蛋白受体、叶酸均值较正常对照组无显著差异,铁蛋白均值为19.62±11.52 ng/L,明显低于对照组61.14±39.92 ng/L(P<0.05)。维生素B12均值为302.3±108.7 pg/L,明显低于对照组522.4±204.6 pg/L(P<0.05)。按血红蛋白浓度的高低分为3组,1组为Hb<11.0 mg/L,2组为Hb11.0 mg/L^12.0 mg/L,3组为Hb>12.0 mg/L。1组铁蛋白、VitB12和转铁蛋白受体与对照组均有显著差别(P<0.05),2组和3组铁蛋白和VitB12与对照组有显著差别(P<0.05)。结论在诊断妊娠妇女缺铁性贫血中,铁蛋白是估计人体铁状态的最好参数,可溶性转铁蛋白受体测定在怀孕期间没有显著变化,如有功能性铁缺乏会出现sTfe浓度升高。而血清铁是检测缺铁及铁负荷过多疾病的有效指标,但在一定时期内表现不明显,变化不大。Objective To detect parameters of serum iron, ferritin and soluble transferrin receptor and VitiminB12 for evaluating their performance in the assessment of iron deficiency in pregnant women. Methods A total of 73 serum samples obtained from 43 pregnant women with gestation period between 24 to 28 weeks and 30 normal women were measured by immunoassay and chromometry. Resnlts There were no significant differences in the median of serum iron, soluble transferrin receptor and Folie acid between two groups. The median of ferritin(19.62±11.52 ng/L) in pregnant women was lower significantly than the median of ferritin(61.14 ± 39.92 ng/L) in normal women( P 〈 0.05). The median of VitiminB12(302.3± 108.7 pg/L) in pregnant women was lower significantly than the median of VitiminBl2 (522.4 ± 204.6 pg/ L) in normal women( P 〈 0.05). 43 pregnant women were divided into three groups according to different concentration of hemoglobin. There were significant differences in the median of ferritin.VitiminB12 and soluble transferrin receptor between first group(Hb 〈 11.0 mg/L) and control( P 〈 0.05). There were significant differences in the median of ferritin and VitiminBt2 between second group(Hb 11.0mg ±12.0mg/L) and control(P 〈 0.05), so did third group( Hb 〉 12.0 rag/L). Conclusion Serum fcrrintin parameter in diagnosis of pregnant women with iron deficient anemia was more valuable in assessment of amount of iron in human body. Serum soluble transferin receptor parameter was no change significantly in pregnant women in gestation period. The concentration of sTfe in serum could be increased in case of functional iron deficiency in pregnant women. The serum iron parameter detected was more valuable in the assessment of disease with iron deficiency or iron overplus accumulation, but it could be not changed in certain stage.
