带血供胫骨骨膜移位术的临床应用

摘　　要：目的：解决胫骨中、下段骨不连、骨缺损和陈旧性骨折等治疗难题。方法：应用胫骨筋膜骨膜瓣、带胫前血管的骨膜瓣和节段血供胫骨骨膜瓣移位术三种手术方法，治疗胫骨骨不连接４例，陈旧性胫骨骨折３例，新鲜骨折３例，左胫骨上段骨纤维异样增殖症切除后骨缺损１例。结果：９例病人于１２～１６周骨愈合，２例骨不连接病人于术后２１周骨愈合。结论：带血供胫骨骨膜瓣移位术操作简单，不需吻合血管，可在一个切口中完成，病人痛苦小，骨愈合快。节段血供胫骨骨膜瓣移位术不牺牲小腿主要血管。Objective:The aim of this study is to explore the operating technique of repairing clinical fracture of nonunion or delayed union,and bone defect at the junction of middle and distal tibia,which are difficult problems in clinic.Methods:4 cases of 11 patients suffering from nonunion,3 cases from delayed union of tibial fractures and 3 cases of fresh fractures of the tibia,and 1 patient in a case of abnormal fibroplasia of the tibia were treated by using tibial fasciperiosteal flaps,periosteal flaps which are supplied with anterior tibial artery or sectional blood vessels.Results:The bone fractures and defect reunited in 9 cases of the patients in 12～16 weeks,and other 2 patients in cases of nonunion in 21 weeks postoperatively.Conclusions:It is a simple operation to transfer the tibial periosteal flap for repairing the fracture and defect of the tibia.Microvascular anastomosis do not need to be used;The bone fracture and defect repaired by this technique can be cured during a shorter time.The operation can be performed only by one incision.The main blood vessel of lower limb do not lost when the periosteal flap supplied with sectional blood vessels are used.
