低分子肝素治疗重度子痫前期患者效果的研究

摘　　要：目的:探讨重度子痫前期患者应用不同剂量低分子肝素的临床效果及安全性。方法:随机将重度子痫前期62例分为3组:A组(大剂量组),在常规治疗的基础上加用低分子肝素5000U/d;B组(小剂量组),加用低分子肝素2500U/d;C组,常规治疗组。通过临床症状、体征及辅助检查的变化判断各治疗组的治疗效果及对母儿的影响。结果:3组患者纳入研究时临床指标无差异(P>0·05);加用低分子肝素组(A、B两组)的临床疗效评分显著高于常规治疗组(C组)(13·43±2·75,11·95±3·46vs9·43±3·81(P<0·05));A组的平均得分高于B组,但统计学分析无显著差异(P>0·05);3组孕妇的肝肾功能、胎儿宫内状况、分娩方式及产后出血量等均无显著差异(P>0·05)。结论:低分子肝素有助于改善重度子痫前期孕妇的治疗效果;低分子肝素对母儿安全。Objective： To study the effectiveness and safety of low molecular weight heparin used in women with severe pre-eclampsia. Methods：Sixty-two women with severe pre-eclampsia were divided into three groups randomly, all of them were given common clinical scheme. Additionally, group A were given low molecular weight heparin 5000U IH daily, and group B 2500U IH daily for 5 days both. Then the clinical symptoms and the results of laboratory tests were analyzed and scored to evaluate the effectiveness, side effects of low molecular weight heparin on the mothers and their fetuses. Results：The conditions of the women in the three groups had no differences when they were arranged to the study. After the treatment, women in group A and B had a better results than group C （scored 13.43 ± 2. 75,11.95 ± 3.46,9. 43 ± 3.81 separately） ;The results of the laboratory test about the function of liver and kidney of the pregnant women were not affected by the low molecular weight heparin,and the state of the fetuses were the same in all groups. Conclusion： Low molecular weight heparin have some clinical effects on severe pre-eclampsia women and it is safe for the mothers and their fetuses.
