妊娠高血压综合征患者血小板功能的研究

摘　　要：目的探讨妊娠高血压综合征(妊高征)患者血小板聚集(PAgT)、释放试验及血小板α-颗粒膜蛋白(GMP- 140)的测定及其临床意义。方法应用阻抗法与荧光法同步测定 PAgT 及 ATP 释放反应；应用 ELISA 法测定血浆中血小板 GMP-140。结果妊高征患者 PAgT、ATP 释放量及血浆 GMP-140含量较正常非孕组明显升高,差异有统计学意义(P＜ 0.05或 P＜0.01)；与正常晚孕组比较,妊高征各组患者 PAgT、血浆 GMP-140含量活性显著增高(P＜0.05或 P＜0.01),且病情越重增高越明显；而 ATP 释放量在晚孕组及妊高征各组则无明显差异。轻、中、重度妊高征 PAgT 与血浆 GMP-140含量水平呈直线正相关关系(r=0.653,P＜0.05；r=0.737,P＜0.05；r=0.778,P＜0.01)。结论妊高征患者由于血小板处于过度活化状态,引起凝血功能亢进,可能是导致妊高征发病和引起临床症状的原因之一。Objective To explore the changes and clinical significance of blood platelet aggregation test （PAgT） ,platelet release reaction and platelet alpha granule membrane protein （GMP-140） levels in patients with induced pregnancy hypertension syndrome. Methods PAgT and the release of platelet ATP were measured simultaneously using a whole-Blood Lumi-Aggregometer,and plasma GMP-140 levels were determined by ELISA respectively in 50 non-pregnancy （control group ）, 50 normal pregnancy women （normal pregnancy group） and 85 patients with PIH （PIH group）. Results In normal late gregnancy group and PIH group, PAgT and release of platelet ATP and plasma GMP-140 levels were significently higher than those of control group ;In PIH group PAgT and GMP-140 were significently higher than those of normal pregnancy group but platelet ATP please reaction did not. There was a positive correlation between the PAgT and plasma GMP-140 levels in mild, moderate and severe PIH group （r = 0.653, P 〈 0.05 ; r = 0.737, P 〈 0.05 ; r = 0. 778, P 〈 0.01 respectively）. Conclusion There was an abnormal platelet activted in patients with pregnancy induced hypertension, which may lead the coagulation system presenting an abnormal higher level.
