摘 要:目的 运用彩色组织多普勒显像观察判断左室舒缩运动中心的位置。方法 运用彩色组织多普勒显像技术观察15例正常人左室整体室壁舒缩运动,取心尖五腔心和心尖左室长轴切面,在收缩早中期和舒张中期室壁运动彩色显像反转处心肌中层与二维图像扇尖做一连线,垂直该连线再作一直线与参考左室长轴相交,该交点认为是运用彩色组织多普勒估测的左室舒缩运动中心。结果 正常人左室收缩运动中心位置在参考左室长轴的67、2%±2、1%(64.8%~70、9%)位点处,舒张期运动中心在69.7%±2、8%(64、6%~71、5%)位点处,且随着心室收缩、舒张左室长轴的变化,该中心的位置保持相对不变。结论 彩色组织多普勒显像作为一种无创、实时、直观的成像方式,可估测以往需通过有创性方法才能观察到的左室运动中心位置,对左室壁整体运动状况提供一宏观评价,具有一定临床价值。Objective To determine the center of left ventricular (LV) segmental wall motion by color tissue Doppler imaging. Methods A total of 15 healthy individuals involved in this study were assessed by color tissue Doppler imaging. The images in early and mid-systole and mid-diastole were analysed frame by frame off-line. A line was drawn to pass through the site at segmental wall where color changed and the tip of scan imaging. Another line that was perpendicular to the first line as above was drawn to intersect the line from the anterolateral aortic valve edge to the apex. The intersection was considerated as the center of the left ventricular segmental wall motion. Results The center of the LV segmental wall motion was at a point 67.2%±2.1 % of the distance from the anterolateral aortic valve edge to the apex during systole and at 69.7% ±2.8% point during diastole, The point did not change its location quite markedly independent of the variation of LV long axis from systole to diastole. Conclusions Color tissue Doppler imaging can determine the center of LV segmental wall motion quite accurately that only could be done by invasive method at the past.
