摘 要:目的对比研究男性代谢综合征(MMS)患者与健康男性红细胞膜胰岛素受体(EIR)数目及亲和力差异及其与血浆游离脂肪酸(FFAs)、胰岛素敏感指数(ISI)的关系;观察油酸对两组人群离体红细胞的干预影响,初步探讨FFAs致胰岛素抵抗的机理。方法测定30例MMS患者与30例健康男性的空腹血糖(FBS)、空腹胰岛素(FINS)、FFAs,EIR的数目和亲和力;各组随机抽取10例,分离洗涤红细胞,以油酸或平行对照组液体孵育,测定两组干预前后EIR的数目和亲和力。结果①MMS组高、低亲和力EIR数目显著低于正常对照组(P<0·05),高亲和力EIR数目与FFAs显著负相关(P<0·05),ISI与高亲和力EIR数目显著正相关(P<0·05)。③油酸干预后,正常对照组中高、低亲和力EIR数目较干预前明显下降,经平行对照组校正后,差异仍有显著性(P<0·05);MMS组中高、低亲和力EIR数目较干预前明显下降,经平行对照组校正后,仅高亲和力EIR数目变化值差异具有显著性(P<0·05)。各组干预前后高、低亲和力EIR的亲和常数均无显著性改变。结论①MMS患者存在胰岛素抵抗。②MMS患者EIR的高亲和力位点数下降与血浆中FFAs升高密切相关,并间接反映了胰岛素抵抗程度。③油酸可能使胰岛素受体结合胰岛素能力下降,导致胰岛素抵抗。Objective To compare insulin receptor in erythrocytes between male with metabolic syndrome and healthy male and study the effects of oleic acid. Methods O We measured FBS, FINS, FFAs, the number and affinity constant of high or low affinity receptors in erythrocytes in 30 male with metabolic syndrome and 30 healthy male. 0 Erythrocytes from 10 members picked up randomly from each group were purified by washing and centrifugation. Each sample was incubated with or without OA (0. 4 mmol/L OA/1 % BSA). The number and affinity constant of high or low affinity receptors in erythrocytes were measured by radioanalysis methods before and after oleic acid incubation. Results ① The number of high or low affinity receptors in erythrocytes from male with metabolic syndrome were less than those from healthy controls significantly. ②In metabolic syndrome group, there was a marked negative correlation between the number of high affinity receptors in erythrocytes and serum free fatty acids and a marked positive correlation between insulin sensitive index and the number of high affinity receptors in erythrocytes. ③ The number of high affinity receptors in erythrocytes in two groups were reduced significantly after incubated with oleic acid, while no changes in affinity constant of high or low affinity receptors was found. Conelusion ① Men with metabolic syndrome have insulin resistance.② The numbers of high affinity receptor in erythrocytes from male with MS were in related to the levels of serum of FFAs and degree of insulin resistance indirectly. ③ Oleic acid reduced binding ability of insulin receptors leading to insulin resistance.
