摘　　要：目的探讨经门静脉注射供体活细胞对移植物慢性排斥反应(CR)及长期存活质量的影响。方法建立大鼠心脏移植模型,随机分成5组A组为对照组,B组给予环孢霉素A(CsA)每日15mg/kg体重灌胃,C组于术中经门静脉注射供体脾细胞1×108个,D组注射脾细胞和骨髓细胞各1×108个,E组在D组基础上加注肝细胞1×108个。观察生存时间,对长期存活的移植物进行组织形态学分析及半定量逆转录-聚合酶链反应(RT-PCR)检测。结果D组和E组移植心长期存活(>90d),功能(2.3±0.5)级,受累的血管数<50%,血管评分1期,与B组及C组比差异有统计学意义(P<0.05),血管病变和纤维化相关因子表达明显减低。结论经门静脉注射两种或两种以上供体活细胞可明显减轻慢性排斥反应并改善移植物长期生存质量。Objective To investigate the effects of donor live cell injection via portal vein on chronic rejection of heart grafts and to evaluate the quality of the long-survived grafts. Methods After the establishment of heart transplantation model in rats, the recipients were randomized into 5 groups. Recipients in group A were given no treatment as control. Recipients in group B were given CsA 15 mg/kg/ d orally. Recipients in group C were given 1×10^8 donor spleen cells via portal vein. Recipients in group D were given 1×10^8 donor spleen cells and bone marrow cells of the same dosage. Recipients in group E were given 1×10^8 donor spleen cells, bone marrow cells and liver cells respectively. The grafts survival time was monitored. The long-survived grafts were analyzed morphologically and the expression of VEGF, bFGF and ET-1 was assessed by reverse transcription-polymerase chain reaction. Results Injection of two or more than two kinds of donor ceils could prolong the heart graft survival time （ 〉90 days） and the ruction of heart grafts remained grade 2.3 ± 0.5. The long-survived grafts showed light vasculopathy with blood vessel valued phase 1 and significant difference was observed as compared with group B or C （P〈0.05）. The expression of VEGF, bFGF and ET-1 in combined injection groups was significantly reduced. Conclusion Injection of two or more than two kinds of donor live cells could alleviate chronic rejection and markedly improve the quality of long-survived grafts.
