摘 要:目的:进一步验证糖尿病患者眼前房液中血管内皮生长因子(VEGF)变化的临床意义,研究VEGF在糖尿病视网膜病变(DR)发病机制中的作用。方法:收集糖尿病不伴有眼底改变与患有糖尿病视网膜病变白内障患者的前房液样本,并以无糖尿病白内障患者的前房液样本作为对照,用ELISA方法测定了前房液中VEGF的含量。结果:对照组10例前房液样本VEGF含量为0。糖尿病患者无眼底改变组中4例VEGF含量为0,12例VEGF含量为(108.8±40.3)ng/L;单纯型DR组中有2例VEGF含量为0,7例VEGF含量为(247.7±79.1)ng/L;增生型DR组6例VEGF含量为(552.3±130.9)ng/L。和对照相比糖尿病患者眼前房液中VEGF的含量均显著增加(P<0.01),并且随着DR病情的进展以及病程的延长而增高(P<0.05);增生型DR组的糖尿病病程明显长于无眼底改变组、单纯型DR组(P<0.05)。结论:VEGF积极参与了DR的发生发展,并与DR后期新生血管形成关系密切。Objective: To explore the changes of the level of VEGF in the aqueous humor of diabetic patients and to investigate the role of VEGF in the pathogenesis of diabetic retinopathy (DR). Methods: The aqueous humor sample of diabetic patients with cataract or without retinopathy were collected. The aqueous humor sample from the non-diabcfic patients with cataract were chosen as the control. The content of VEGF in the aqueous humor was determined by using the ELISA technique. Results: The level of VEGF in the aqueous humor samples from the control and 4 out of 16 cases of diabetic patients without retinopathy Was undetectable and the level of VEGF of the other twelve cases of diabetic patients without retinopathy was 108.8 ± 40.3 ng/L. The level of VEGF in aqueous humor was undetectable in 2 cases of bachground diabetic retinopathy (BDR), and the level of VEGF of the other seven cases was 247.7 ± 79.1 ng/L. The level of the VEGF in six cases of proliferative diabetic retinopathy (PDR) was 552.3 ± 103.9 ng/L. Compared with the control, the level of VEGF in the aqueous humor of diabetic patients increased significantly (P 〈 0.01 )and was associated with the severity degree and prolongation of DR(P 〈 0.05 ). The process of diabetes in PDR group is obviously longer than that of diabetes without retinopathy and BDR (P 〈 0.05 ). Conclusion: VEGF plays an active role in the generation and development of DR and has a close relation with the neovascularization of the advanced stage of DR.
