摘  要:目的研究2型糖尿病患者血清维生素C和维生素E水平与胰岛素抵抗(IR)间的关系。方法选取初诊2型糖尿病患者61例,健康对照50例,测定身高、体重、腰围、臀围及空腹血清维生素C、维生素E、胰岛素、血糖、血脂及糖化血红蛋白水平。计算所有受试者体质指数和腰臀比,应用稳态模式胰岛素抵抗指数(HOMA-IR)作为评价胰岛素抵抗指标,对以上指标及临床特征进行对比分析。结果①糖尿病组血清维生素C、维生素E水平明显低于对照组,且随血糖水平升高而降低。②糖尿病组体质指数、腰臀比、总胆固醇、甘油三酯、低密度脂蛋白及胰岛素水平明显高于对照组,高密度脂蛋白则低于对照组。③相关分析显示胰岛素抵抗指数与腰臀比、甘油三酯、维生素E水平、体质指数密切相关,与维生素C水平无显著相关。结论2型糖尿病患者血清维生素C和维生素E水平降低,其中IR者的维生素E水平更低。Objective To investigate the relationship between insulin resistance and vitamin C, vitamin E in type 2 diabetes mellitus. Methods Sixty-one patients with type 2 diabetes and 50 normal subjects were studied. Body height, body weight, waistline, hip girth, serum vitamin C and vitamin E, fasting insulin(Fins), fasting plasma glucose(EPG),plasma lipids and HbA1C were measured. Body mass index(BMI) and waist-hip ratio (WHR) were calculated, homeostasis model assessment was applied to estimate the degree of insulin resistance. At last these objects were analysed and compared. Results (1)Vitamin C and vitamin E levels in diabetic group were much lower than control, and reversely related to FPG and HbA1C. (2)BMI, WHR, total cholesterol(TC) ,triglycerid (TG), low-density lipoprotein cholesterol(LDL-C) and Fins in diabetic group were much higher than controls, high-density lipoprotein cholesterol(HDL-C) were much lower than control. (3)Correlated analyses showed WHR, TG and BMI were independent risk factors for IR, vitamin E was proctective factor for IR.Conclusion The concentrations of serum Vitamin C and vitamin E in type 2 diabetes significantly decreased, the level of vitamin E was much lower in IR patients.
