3种填塞材料在鼻中隔矫正手术中效果的比较

摘　　要：目的探讨鼻中隔矫正手术病人使用3种不同鼻腔填塞材料时鼻分泌液中肿瘤坏死因子α(TNF-α)及白蛋白水平的变化,并分析其与鼻腔黏膜炎性变化的关系,从而寻找理想鼻腔填塞材料并提供理论依据。方法将30例鼻中隔矫正手术病人随机分为3组,术后分别使用3种不同鼻腔填塞材料:凡士林纱条,红霉素软膏纱条和sorbalgon;分别在术前及术后48h、96h取鼻分泌液,采用酶联免疫吸附法(ELISA)检测TNF-α的水平,全自动生化分析仪检测白蛋白的含量,比较3组病人鼻分泌液中TNF-α及白蛋白水平的变化情况。结果术前3组病人鼻分泌液中TNF-α和白蛋白水平均无差异(P=0.85;P=0.45)。术后48h和96h 3组病人鼻分泌液中TNF-α和白蛋白均有显著差异,两组数据均以sorbalgon组病人的检测值最低,红霉素软膏组病人的检测值居中,凡士林纱条组病人的检测值最高(P<0.05)。结论sorbalgon组病人术后鼻分泌液中TNF-α和白蛋白水平低于红霉素软膏纱条组和凡士林纱条组;表明sorbalgon在减轻鼻黏膜炎症方面优于其它两组,因此,sorbalgon为理想的鼻中隔矫正术后鼻腔填塞材料。Objective To explore the changes of TNF-α and albumin in nasal flnid of nasal septum deviation patients packing with three kinds of different stuffs after corrective operation, and analyze the relationship between nasal mucosa inflammation and the level of TNF-α and albumin. Methods Thirty nasal septum correction patients were divided in 3 groups, three kinds of nasal packing stuffs （Vaseline, erythromycin ointment and sorbalgon） were used after operation. Examining the level of TNF-α and albumin in nasal fluid before operation,48h and 96h after operation respectively; Observing the agglutination of nasal mucosa at 96h after operation. Results The level of TNF-α and albumin in nasal fluid before operation had no difference among three groups（P =0.86; P =0.45）;The level of TNF-α and albumin had difference at 48h and 96h after operation with statistical significance, the level of TNF-α and albumin were lowest in sorbalgon group, middle in erythromyein ointment group and highest in Vaseline group at 48h and 96h after operation（ P 〈 0.05）. Conclusion The levels of TNF-α and albumin in sorbalgon group are lower than those in other two groups, which show that sorbalgon is better in reducing inflammation; therefore, sorbalgon is an ideal nasal packing stuff.
