摘 要:目的观察移植前低钾右旋糖苷液(LPD)二次灌洗对供肺再灌注损伤的保护作用。方法24只大鼠随机分为对照组(n=12)和实验组(n=12),对照组供肺经LPD液灌洗后于4℃保存18 h,取右肺组织测丙二醛(MDA)、髓过氧化物酶(MPO)、白细胞介素-8(IL-8),建立离体肺再灌注模型,左肺灌注30 m in;实验组供肺灌注前用LPD液再灌洗一次,余同对照组。灌注期间每隔10m in测定供肺静脉流出血氧分压(PaO2)、供肺平均肺动脉压(PaP)、气道峰压(PawP),灌注结束后测肺组织丙二醛(MDA)、髓过氧化物酶(MPO)、白细胞介素-8(IL-8)的含量。结果实验组供肺右肺组织MDA、MPO、IL-8含量明显低于对照组,差异有统计学意义(P<0.01);再灌注30m in后实验组供肺PO2明显高于对照组,PaP、PawP明显低于对照组,差异有统计学意义(P<0.01);灌注后供肺组织MDA、MPO、IL-8含量,实验组明显低于对照组,差异有统计学意义(P<0.01)。结论移植前LPD液二次灌洗可以减少供肺组织内MDA、MPO、IL-8含量,减轻供肺的再灌注损伤,改善供肺的功能。Objective To studied the effects of administering a second rinse solution of low-potassium-dextran (LPD) before reperfusion on the donor lung of rat after hmg time, cold ischemie preservation. Methods Twenty-four rats were randomly divided into control group ( n = 12) and trial group ( n = 12). Donor lungs of control group were flushed with LPD solution. The levels of mahmaldehyde ( MDA), myeloperoxidase (MPO) and interleukin-8 ( IL-8 ) were tested in the tissue of right lung and the left lungs were reperfused with venous blood for 30 minutes. The partial pressure of nxygen ( PaO2 ), PaP anti PawP were measured at every 10 minutes intervals during reperfusion. After reperfusion, the MDA, MPO activity and interlakin-8 (IL-8) were determined in the lung tissue. Results The levels of MDA, MPO and IL-8 decreased significantly in right lung of trial group than those in control group ( P 〈 0. 01 ). After reperfusion of 20 minutes, the PaO2, were higher in trial group than that in control group, and PaP, PawP were lower in trial group than those of control group ( all P 〈0. 01 ). After reperfusion, the MDA, MPO anti IL-8 decreased significantly in left lung of trial group than in control group ( all P 〈 0. 01 ). Conclusion A second flush with LPD solution before reperfusion can decrease the levels of MDA, MPO and IL-8, and thus improves the function of donor lungs.
