DSA检查和血管栓塞术在鼻咽及口腔颌面部病变中的应用

摘　　要：目的 探讨数字减影血管成像（DSA）检查和血管栓塞术在鼻咽及口腔颌面部病变中应用的临床价值。方法 经DSA检查证实的10例鼻咽及口腔颌面部富血供病变患者，8例良性病变施行超选择血管栓塞术，2例恶性病变施行超选择血管内灌注化疗及栓塞术。结果2例上／下颌骨、2例舌及颌下区血管瘤，栓塞后痊愈出院。4例鼻咽部纤维血管瘤及1例恶性病变栓塞后3～5天行手术治疗，术中病灶及范围易于辨认和分离，出血很少，术野清楚，瘤体被完整切除。另1例恶性病变介入术后病灶控制至今仍较好。结论 鼻咽及口腔颌面部血管性病变血管栓塞术部分病例可治愈，实体瘤经血管栓塞／灌注化疗及栓塞术后，可有效控制肿瘤生长及术中出血，给彻底切除肿瘤创造了条件，且更具有微创，成功率高，疗效显著，安全，并发症少，可重复操作等优点。Objective To evaluate the clinical value of digital subtraction angiography （DSA） inspection and intravascu lar embolization in nasopharynx, oral cavity and maxillofacial disease. Methods Ten patients with nasapharynx, oral cavity and maxillofacial diseases proved by DSA inspection were plenty of vascular, 8 benign cases of them were treated by superselect arterial embolization, 2 malignant cases were treated by superselect arterial infusion chemotherapy and embolization. Results Two patients with maxillomandibular hemartoma and 2 patients with left tongue, under mandible region hemartorna were cured following embolization. They haven＇t recrudesced so far. Four cases of nasopharyngeal hemangiofibroma and 1 case of malignant disease were operated 3--5 days after embolization. The lesion can be easily identified and separated ,the operative region was clear, the bleeding was reduced during operation,and the tumor body was resected completely. Conclusion Embolization on nasopharynx, oral cavity and maxillofacial region vascular diseases can take the place of surgical operation. It has the merits of microinvasion, high success rate, obvious therapeatic effect, safty, lowcomplications and repeatability. Intravascular embolization, infusion chemotherapy on solid tumor is helpful for restraining the growth of the tumor and controlling the haemorrhage during operation as well as resecting the tumor completely.
