摘  要:目的 通过多层螺旋CT灌注扫描研究肝脏微小转移癌大鼠的肝脏血流动力学变化。方法 22只经脾脏注入Walker-256(细胞数量2×10^7)制备成肝脏微小转移癌的大鼠为实验组;10只大鼠经脾脏注入等量生理盐水为对照组。CT灌注扫描后用斜率法计算得到参数包括:肝动脉灌注量、门静脉灌注量、肝灌注指数、总肝灌注量。病理学检查HE染色证实转移癌的存在。结果 19只大鼠发现的转移灶直径在0.5~6.6mm之间。肝脏微小转移癌的大鼠肝动脉的灌注量远远大于对照组(是对照组的2.25倍);而门静脉灌注量明显少于对照组(是对照组的76.6%)。总肝灌注量没有显著变化。结论 大鼠微小肝转移癌表现为肝动脉灌注量的增加和门静脉灌注量的降低。CT灌注成像技术可以用来评价肝脏的血流动力学变化。Objective To evaluate the hemodynamic changes of the liver caused by hepatic micrometastasies in rats with multislice computed tomography (CT). Methods laver micrometastases were produced in 22 SD rats by transspleenic injecting 2 × 10^7 Walker-256 cells. Ten experimental control rats were injected with saline solution. The time-attenuation curves of the aorta, portal vein, and the liver were used to calculate liver perfusion by gradient method designed for the dual blood supply. Results The micrometastases (diameter, between 0.5 mm and 6.6 mm) were seen in 19 rats. Hepatic arterial perfusion was 2.25 times in the rats with micrometastases than that in the control rats. Portal hepatic perfusion was 76. 6% lower in the rats with micrometastases than that in the control rats. Total hepatic blood flow had no significant change. Conclusion Liver micrometastases in rats generate changes in liver perfusion that can be detected with CT. Small liver metastases in rats can be found decreasedlower the portal perfusion and higher increased the arterial perfusion. Perfusion CT techniques can be used to evaluate the hemodynamic changes of the liver.
