摘  要:目的:为前臂桡侧头静脉-皮神经营养血管远端蒂复合瓣设计提供解剖学基础。方法:动脉灌注红色乳胶成人上肢标本,解剖观测头静脉-前臂外侧皮神经下1/3段营养血管的来源、分支及其与桡骨膜血管的关系。结果:头静脉-前臂外侧皮神经下1/3段的营养血管来自:桡动脉皮支,掌浅支皮支,桡骨茎突返支皮支和桡动脉肌间隙骨皮支。上述诸支血管发皮支、筋膜支、骨膜支、神经-浅静脉营养血管,形成皮神经-头静脉血管链以及深、浅筋膜和骨膜血管网。结论:前臂桡侧缘头静脉-皮神经营养血管与肌、骨、皮营养血管同源,以桡骨茎突返支为蒂的远端蒂复合瓣,旋转轴点在腕关节平面,可用于手部远处组织缺损修复。Objective: To provide the anatomic basis for designing compound flap of distally based pediele with cephalic vein-cutaneous nerve at radial margin of forearm. Methods. The origins and branches of nutritional vessels of the lower 1/3 segment of the cephalic vein-lateral antebrachial cutaneous nerve of forearm and its relationship with periosteum vessels of radius were observed on adult upper limb specimens perfused with red emulsion in artery. Results. The origins of nutrient vessel of the 1/3 inferior segment of the cephalic vein and lateral antebrachial cutaneous nerve was as follows, the cutaneous branches of radial artery, cutaneous branches of superficial palmar branch, cutaneous branches of malleolus radialis recurrent branch. Above mentioned perforating branches included fascia branches, cutaneous branches, nerve and vein nutrition branches. Those nutrient vessels originated longitudinal vessel chain of the lateral cutaneous nerve of forearm-cephalic vein, deep subcutaneous fascia and periosteum vessel network. Concluslon. The nutrient vessels of the cephalic vein of radial margin of forearm and cutaneous nerve have homology with the nutrient vessels of muscles, bone and skin. The compound flap of distally-based pedicled with the malleolus radialis recurrent branches with the rotation point at the wrist joint can be appiled to repair tissues defect of the hand distant parts.
