非Graves甲亢和Graves’病FT_3、FT_4、TSH、TGA、TPO-Ab的变化及意义  

Significance of Changes of Serum FT_3,FT_4,s-TSH,TGA,TPO-Ab Levels in Patients with Non-Graves' Hyperthyroidism and Graves' Disease

摘　　要：目的:探讨FT3、FT4、s-TSH(超灵敏)、甲状腺过氧化物酶抗体(antithyroid peroxidase antibody,TPO-Ab)、抗甲状腺球蛋白抗体(antithyroidglubu lin antibody,TGA)水平在甲亢疾病和G raves’病中的改变及其临床意义。方法:对43例非G raves’病患者和29例G raves’病患者、40例正常人分别检测血清TF3、FT4、s-TSH、TPO-Ab、TGA的水平。结果:非G raves’病甲亢患者的FT3(15.01±11.01pg/m l)、FT4(38.30±19.82pg/m l)均明显高于正常人(分别为4.27±0.73pg/m l,16.16±2.58pg/m l,P<0.01),s-TSH(0.25±0.25m IU/L)明显低于正常人(2.24±1.26m IU/L,P<0.01),TPO-Ab(1319.74±1037.78 IU/m l)明显高于正常人(0.01±0.01,P<0.01)、TGA(33.39±22.43%)明显高于正常人(6.76±2.01,P<0.01)。G raves’病患者FT3(15.23±9.57pg/m l)、FT4(38.57±17.39pg/m l)均明显高于正常人(P<0.01),s-TSH(0.79±0.46m IU/L)明显低于正常人(P<0.01),TPO-Ab(2023.24±621.00 IU/m l)明显高于正常人(P<0.01)、TGA(49.72±20.55)明显高于正常人(P<0.01),甲亢患者和G raves’病患者FT3、FT4、s-TSH无明显差别,但TGA和TPO-Ab明显低于G raves’病患者(P<0.05)。结论:FT3、FT4、s-TSH(超灵敏)、甲状腺过氧化物酶抗体(TPO-Ab)、抗甲状腺球蛋白抗体(TGA)在判断甲亢和G raves’病患者中有重要的临床鉴别诊断意义。Objective To investigate the clinical significance of the changes of thyroid - related hormones （FT3, FT4,s - TSH, TGA, TPO - Ab） levels in patients with Graves＇ and non - Graves＇ hyperthyroidism. Methods Serum FT3,FT4, TGA, TPO - Ab （with RIA） and s- TSH （with IRMA） were determined in 43 patients with non- Graves＇ hyperthyroidism, 29 patients with Graves＇ disease and 40 controls. Results In both groups of hyperthyroid patients, the serum levels of FT3 （ 15.01 ± 11.01pg/ml in the non - Graves＇ group and 15.23 ± 9.57pg/ml in the Graves＇ group）,FT4 （38.30 ± 19.82, 38.87 ± 17.39pg/ml）, TGA（33.89 ± 22. 43%, 49.72 ± 20.55% ） and TPO - Ab （ 1319.24 ± 1037.78, 2023.24 ± 621.00IU/ml） were significantly higher than those （ FT3, 6.76 ± 2.01pg/ml, FT4 16.16 ± 2.58pg/ml, TGA 6.76 ± 2.01%, TPO - Ab 0.01 ± 0. 01IU/ml） in the controls （ all P 〈 0.01 ）. The serum s - TSH levels in both the patient groups （0.25 ± 0.25 mlu/L, 0.79 ± 0.46mlu/L） were significantly lower than those （2. 24 ± 1.26mlu/L） in the controls （P〈0.01）. Serum levels of TGA and TPO -Ah in patients with non- Graves＇ hyperthyroidism were significantly lower than those in patients with Graves＇ disease, but the FT3 ,FT4 and s -TSH levels in both groups were not significantly different from each othen. Conclusion In this study, serum TGA and TPO - Ab levels were lower in patients with non - Graves＇ hyperthyroidism than those in patients with Graves＇ disease.
