摘　　要：目的:探讨胰岛素样生长因子结合蛋白-1(IGFBP-1)对延期妊娠引产时机选择的意义。方法:选择延期妊娠孕妇79例,检测宫颈分泌物IGFBP-1的同时进行宫颈Bishop评分,观察IGFBP-1与Bishop评分的相关性。根据IGFBP-1检测结果分为阳性组和阴性组,根据Bishop评分结果分为宫颈成熟组和未成熟组,按催产素点滴常规进行引产。观察各组引产效果成功率,评价IGFBP-1与Bishop评分对预测引产成功的敏感性、特异性、阳性预测值和阴性预测值。结果:IGFBP-1与Bishop评分呈正相关,其总体相关系数为0·98(P<0·05);IGFBP-1阳性组84h内引产成功率明显高于阴性组,差异有显著性(P<0·001);宫颈成熟组与未成熟组84h内引产成功率差异无显著性(P>0·05);Bishop评分≤3分且IGFBP-1阴性的10例孕妇无1例引产成功;IGFBP-1预测引产成功的敏感性、特异性、阳性预测值和阴性预测值优于Bishop评分。结论:IGFBP-1可作为延期妊娠引产时机选择的重要指标,IGFBP-1结合Bishop评分可提高延期妊娠引产成功率。Objective： To explore the significance of the choice of insulin - like growth factor binding protein 1 （ IGFBP - 1 ） on the induced labor of post - term pregnancy. Methods： 79 cases of post - term pregnancy gravida were analyzed, Measured IGFBP - 1 of cervical secretion and marked cervix at the same time to determine whether IGFBP - 1 was associated with cervix mark. Divided them into posi- tive group and negative group at the base of the result of IGFBP - 1 ; into cervical maturity group and cervical immaturity group at the base of Bishop marks. Did induced labor by infusion routine oxytocin. Studied the effect of induced labor of different groups. Evaluated the sensitivity, specificity, positive predictive value and negative predictive value of IGFBP - 1 and Bishop mark on predicting the success rates of induced labor. Results： There was positive correlation between IGFBP - 1 and Bishop mark, the total correlation coefficient was 0. 98. （ P 〈 0. 05 ） Within 8d hours the success rate of induced labor of lGFBP - 1 positive group was higher than that of lGFBP - 1 negative group （ P 〈0. 001 ） There was no significant difference of the success rate of induced labor between cervical maturity group and cervical immaturity group. （ P 〉 0. 05 ） . There was no one of 10 cases whose Bishop mark no more than 3 mark and IGFBP - 1 negative. On the sensitivity, specifwity, positive predictive value and negative predictive value by determination of IGFBP - 1 was better than by Bishop mark to detect the success rate of induced labor. Conclusion： IGFBP - 1 can be an important predictor on the opportunity choice of the induced labor of post - term pregnancy and increase the success rate of induced labor combined with Bishop mark.
