酚磺乙胺用于减少剖宫产产后出血的临床研究

摘　　要：目的:观察酚磺乙胺减少剖宫产产后出血的临床效果。方法:选择急症剖宫产患者90例,随机分为A、B、C三组,各组均30例,胎儿娩出后均宫体注射缩宫素20U。A组于胎头娩出后即给静脉滴注酚磺乙胺2g,并含化卡孕栓1mg;B组胎头娩出后即含化卡孕栓1mg;C组静脉点滴催产素20U。结果:①术中出血量,A组283·70±51·98ml,B组324·90±74·51ml,C组400·30±118·31ml,C组与A、B组差异有统计学意义(P<0·01),A、B组差异无统计学意义(P>0·05);②术后2h及24h出血量,两两比较在统计学上均有显著性差异(P<0·05);③产后出血率,A组(16·67%)、B组(56·67%)、C组(70·00%),三组比较有显著性差异(P<0·001),两两比较A与B、C组有显著性差异(P<0·001),B与C组无显著性差异(P>0·05)。结论:剖宫产术联合应用酚磺乙胺+缩宫剂,能有效地降低剖宫产产后出血率,其疗效优于单用缩宫剂。Objective： To observe the clinical effect of etamsylate used in reducing postpartum hemorrhage after cesarean section. Methods： 90 cases underwent cesarean section for emergency were randomly and averagely divided into three groups ： all cases were injected 20 U oxytocin to uterus immediately after delivery. Group A were given etamsylate 2 g intravenous injection and PGF2a 1 mg orally. Group B were given PGF2a 1 mg orally. Group C were given oxytecin 20 U dripping intravenously, Results： ①In hemorrhage volume, group A 283. 7 ±051.98 ml, group B 324. 90 ±74. 51 ml, group C 400. 30 ± 118. 31 ml; there were significant differences between group C and B and between group C and A （P 〈0. 01 ）, without obvious difference between group B and A （P 〉0. 05） ; ②The hemorrhage volume 2 and 24 h after operation presented obvious difference between any two groups （ P 〈 0. 05 ） ; ③In rate of postpartum hemorrhage, group A 16. 67.%, group B 56. 67%, group C 70. 00%, with significant difference among three groups （P 〈0. 001 ） , and between group C and B and between group C and A （ P 〈 0. 001 ）, but without significant difference between group B and A （ P 〉 0. 05 ） . Conclusion： To combine the cesarean section, etamsylate and PGF2a could effectively reduce the rate of postpartum hemorrhage. The therapeutic effect is better than that of single PGF2a.
