颈动脉超声及血脂分析对脑梗死的临床评价

摘　　要：目的研究颈动脉粥样硬化及血脂水平对脑梗死的临床意义。方法对180例脑梗死病人及70例健康体检者行颈动脉超声及血脂分析检查。根据颈动脉超声检测结果将不同程度的颈动脉粥样硬化脑梗死病人分成A、B、C三组,并对不同程度的颈动脉粥样硬化与脑梗死病人的NIHSS评分及总胆固醇(TC)、甘油三酯(TG)水平与不同程度的颈动脉粥样硬化作对比分析。结果脑梗死组的颈动脉粥样硬化程度均较健康对照组严重,但随颈动脉粥样硬化的加重,A、B、C三组脑梗死病人的NIHSS评分无明显差异;脑梗死组的血TC、TG水平均显著高于健康对照组,且随颈动脉粥样硬化的加重,TC、TG水平逐渐升高。结论不同程度的颈动脉粥样硬化与脑梗死发病有关,但不能预示脑梗死的病情;血TC、TG水平与颈动脉粥样硬化相关。Objective To investigate the clinical significance of different degree of carotid atherosclerosis and various level in blood lipids to cerebral infarction. Methods One hundred and eighty cerebral infarction patients and 70 healthy examination persons were measured by carotid sonography and examined the level of blood lipids which included the total cholesterol （ TC ） and blood triglyceride（TG）. One hundred and eighty cerebral infarction patients with different degree of carotid athcrosclerosis were divided into three groups as A, B and C, according to their degree of carotid atherosclerosis, and compared the different degree of carotid atherosclerosis with the cerebral infarction paticnts NIHSS value and the level of blood lipids with different degree of carotid atherosclerosis. Results The degree of carotid atherosclerosis in cerebral infarction patients were significantly higher than healthy examination persons, but no significant difference in NIHSS value of group A, B and C. The level of blood lipids in cerebral infarction patients were much higher than the healthy examination persons. And with the higher degree of carotid atherosch,rosis,the higher the level of blood lipids was. Conclusion The degree of carotid atherosclerosis has positive correlation to cerebral infarction ,but was negative significance in seriousness of cerebral infarction,while the level of blood lipids was significantly related to the carotid atherosclerosis.
