子宫切除手术途径对组织损伤的评估

摘　　要：目的:评估和比较腹腔镜手术与常规子宫切除对组织损伤的程度。方法:选择2003年2月至2004年9月期间81例有妇科手术指征的患者,分别行腹腔镜下子宫切除术(LH)29例,开腹子宫切除术(AH)36例及阴式子宫切除术(VH)16例。在手术当日早6时和术后24小时采集患者静脉血做标本检测IL-6、TNF-α和CRP。结果:AH组检测的IL-6、TNF-α和CRP明显高于LH组和VH组(P<0．01),但LH和VH 两组之间无显著差异(P>0．05)。结论:腹腔镜手术对免疫功能的抑制程度轻,说明腹腔镜手术对机体的创伤较开腹手术小。Objective： To evaluate and compare the effects of laparoscopic and conventional hysterectomies on tissues damage. Methods： 81 patients with hysterectomy indication were selected from Feb 2003 to Sep 2004, 29 cases were underwent laparoscopic hysterectomy（ LH）, 36 cases were underwent abdominal hysterectomy（ AH）, 16 cases were underwent vaginal hysterectomy（ VH ）. Interleukin-6 （ IL-6 ）, INF-α and C-reactive protein（CRP） from venous serum were detected at Am 6 on operation day and post-operation 24 hours. Results：The levels of IL-6,INF-α and CRP in AH group are significant higher than that in LH group and VH group（ P〈0. 01） ,there were not significant difference between LH group and VH group（P〉0.05）. Conclusions： Laparoscopic operation caused less damage on inhibition immune funetion; the tissue damage of laparoscopic operation is less than that of abdominal operation.
