摘　　要：目的探讨腹部闭合性损伤中胰腺损伤的诊断和治疗。方法分析我院近10年来收治的69例确诊为胰腺损伤患者的临床资料。结果术前B超检查阳性率为32.1%,CT检查阳性率为22.5%,腹腔穿刺液淀粉酶(Ams)升高占54.1%,血Ams升高占59.4%,尿Ams升高占63.8%,69例保守治疗6例,手术治疗63例,死亡11例,病死率为17.5%。结论腹部闭合性损伤中胰腺损伤术前诊断较困难,外科医生要提高对胰腺损伤的警惕性,术中认真探查,视胰腺损伤的程度及合并伤选择合理的术式,少数患者可以保守治疗。Objective To investigate the diagnoses and treatment of blunt pancreatic injury （BPI）. Methods A retrospective analysis was made on the clinical date of 69 cases treated in our hospital during 10 years. Results The positive diagnosis rate was 32. 1% for BUS examination preoperatively, but 22. 5% for CT scannings. The Ams in the ascite, blood, urine were examined, they were increased. In the 69 cases, 6 cases received nooperative treatment, 63 cases received operation, 11 cases died, the mortality was 17.5%. Conclusion It is difficult to diagnose BPI before the operation, so the surgeon must enhance the vigilance and explore carefully. Appropriate methods should be chosen according to the injury degree, a few cases can receive conservative treatment.
